Abstract

O hipopituitarismo pós-traumático é uma sequela relativamente frequente do traumatismo crânio-encefálico todavia, é habitualmente subvalorizada. Dada a elevada incidência mundial do traumatismo crânio-encefálico e das suas consequências clínicas agudas e crónicas, é fundamental diagnosticar as comorbilidades a que este pode estar associado, sendo o hipopituitarismo pós-traumático uma potencial causa tratável de morbilidade nas vítimas de traumatismo crânio-encefálico. Deste modo, esta revisão da literatura foi realizada com o objetivo de compreender melhor esta patologia no seu todo, e especialmente para sumarizar as principais recomendações diagnósticas e terapêuticas do hipopituitarismo pós-traumático, assim como as suas limitações. Constatou-se que não existem ainda recomendações validadas prospectivamente em estudos de grande dimensão para o diagnóstico, para o seguimento ou para o tratamento desta patologia. Apesar de existirem alguns consensos publicados, estes não foram introduzidas nas rotinas clínicas, e estão associados a diversos fatores de incorreção e incerteza, de modo que a controvérsia em volta deste tema é ainda substancial. Assim, concluiu-se que a temática do hipopituitarismo pós-traumático merece especial atenção por parte da comunidade científica, devido à sua evidente relevância clínica e epidemiológica, e à facilidade com que se pode obter o seu diagnóstico e tratamento eficientes, com um enorme benefício clínico, bastando para tal, que se estabeleçam normas diretivas que determinem a quem, como, e quando se deve proceder ao diagnóstico e tratamento desta patologia, e que estas sejam introduzidas de forma sistemática na prática clínica

The posttraumatic hypopituitarism is a relatively common sequel of head trauma however, it’s usually undervalued. Given the high worldwide incidence of traumatic brain injury and its acute and chronic clinical consequences, it is essential to diagnose comorbidities that may be associated with it, as post-traumatic hypopituitarism, which is a potentially treatable cause of morbidity in victims of traumatic brain injury. Therefore, this literature review was conducted in order to better understand this disease as a whole and, especially to summarize the main diagnostic and therapeutic recommendations for posttraumatic hypopituitarism, as well as their limitations. It was recognized that recommendations for screening, monitoring and treatment of this pathology, are not validated in prospective large studies yet. Although there are some published consensus, these have not been introduced in clinical routine and, they are associated with several confounding and uncertainty factors, consequently this topic is still surrounded by substantial controversy. So, it was concluded that the subject of post-traumatic hypopituitarism deserves special attention from the scientific community, due to its clear clinical and epidemiological relevance and, the facility of getting an effective diagnosis and treatment, with a huge clinical benefit. To accomplish that, it’s only necessary to stablish guidelines that determine who, how, and when should be screened and treated, and then introduce them systematically in the clinical practice
