Abstract

A hepatite tóxica medicamentosa (DILI) é a causa mais citada para a retirada de medicamentos do mercado e representa um sério problema de saúde havendo já um registo de mais de 100 medicamentos que provocam esta doença. Apesar da sua importância, o diagnóstico da DILI baseia-se em critérios subjetivos dificultando a sua validação e reprodutibilidade. Em termos gerais, a DILI idiossincrática é determinada pela presença de variações genéticas que codificam genes que desempenham papéis centrais nas vias metabólicas e/ou pela resposta imunológica, e pela interação destas variações genéticas com fatores ambientais. Neste trabalho, a literatura relevante relativa a meios de diagnóstico atuais foi revista, dando ênfase aos polimorfismos das enzimas envolvidas nas vias de bioativação (CYP450) e nas reações de destoxificação e transporte como potenciais biomarcadores. Os estudos de associação e suscetibilidade genética à DILI bem como abordagens futuras para o estudo da DILI foram exploradas. Um conhecimento mais detalhado dos genes candidatos envolvidos na DILI irá permitir identificar doentes suscetíveis ao risco de desenvolvimento de DILI, desenvolver ferramentas de diagnóstico mais fiáveis e estabelecer novas estratégias de tratamento

Drug-induced liver injury (DILI) is the most cited cause for drug withdrawal from the market and is a major health problem. Up to now, more 100 drugs were reported to induce DILI. Besides this facts, DILI diagnosis continues to rely on subjective measurements which makes difficult its validation and reproducibility. From a general point of view, idiosyncratic DILI is determined by the presence of genetic variations key players of metabolic pathways and/or by the immune response, and by the interaction between genetic variants and environmental factors. The relevant literature regarding diagnostic tools was revised, giving special attention to the potential of polimorfisms in enzymes participating in the bioactivation pathways (CYP450), detoxification and transport reactions as DILI biomarkers. Genetic and susceptibility studies as future tools for DILI diagnosis were explored. A detailed knowledge about the candidate genes involved in DILI will allow the identification of patients with increased risk of developing DILI, develop new diagnostic tools and establish new treatment stratagies.
