Abstract

A Hipomineralização Incisivo-Molar (MIH) é um defeito do esmalte dentário que afecta os primeiros molares e incisivos permanentes. Vários factores etiológicos são referidos não se sabendo ainda ao certo qual a sua origem específica. Estes parecem, contudo, ocorrer/actuar nos 3 primeiros anos de vida da criança, altura em que se dá a hipomineralizaçao destes dentes. Os dados disponíveis sobre a prevalência desta condição clínica relevante, são esparsos, variando entre os 3,6-25%, na Europa. A MIH é um defeito da translucidez do esmalte que pode ser muitas vezes confundida com Fluorose, Hipoplasia do Esmalte e Amelogénese Imperfecta. É importante fazer um diagnóstico precoce de forma a obter um melhor prognóstico. Tal garante que o risco e as complicações decorrentes da fractura pós-eruptiva do esmalte (susceptibilidade aumentada para a cárie e hipersensibilidade dentária) sejam minimizados. Crianças com MIH necessitam de um maior número de tratamentos dentários e estão muitas vezes sujeitas a experiências de dor e desconforto em repetidas ocasiões, relacionadas com a ocorrência de problemas de gestão comportamental nestes pacientes. A escolha do tipo de tratamento vai depender da severidade do caso, que pode variar de leve a grave, e pode ir desde a restauração com materiais adesivos até à extracção destes. Mais investigação clínica e pesquisa é urgentemente necessária para validar e melhorar muitos dos problemas de gestão dos dentes hipomineralizados. The Molar-Incisor Hypomineralization (MIH) is an enamel defect presenting in first permanent molars and permanent incisors. Several etiological factors are concerned but a specific origin is yet to be known. These seem, however, to occur/act in the first 3 years of a child's life, when the hypomineralization these teeth occurs. The available data on the prevalence of this clinical relevant condition are sparse, ranging from 3.6 to 25% in Europe. The MHI is a translucency defect of the enamel that can often be confused with Fluorosis, Enamel Hypoplasia and Amelogenesis Imperfecta. It is important to make an early diagnosis in order to obtain a better prognosis. This ensures that the risk and complications of the post-eruptive enamel breakdown (increased susceptibility to dental caries and hypersensitivity) are minimized. Children with MIH need a greater number of dental treatments and often experience pain and discomfort on repeated occasions, associated with the occurrence of behavioral management problems in these patients. The treatment choice will depend on the severity of the case, which can range from mild to severe and can vary from restorations with adhesive materials to the extraction of the involved teeth. More clinical research is needed to validate and improve many of the management problems of the hypomineralized teeth.
