Abstract

O alcoolismo do idoso é um problema em crescimento dado o aumento população geriátrica e da necessidade de cuidados que lhe estão associados. Com este trabalho pretendeu-se proporcionar uma visão geral sobre o consumo de álcool em indivíduos com mais de 65 anos através de uma pesquisa bibliográfica realizada na base de dados PubMed e ClinicalKey na biblioteca do Centro Hospitalar da Universidade de Coimbra. Cerca de 50% dos idosos são consumidores diários de álcool, sendo que 1 a 3% da população geriátrica padece de Distúrbio do Uso do Álcool, causa importante de morbi-mortalidade física e psíquica bem como de stress social. O consumo de álcool crónico está associado a lesão de vários órgãos se expressão mais grave no idoso. Apresentam maior vulnerabilidade ao aparecimento de patologia hepática, maior risco cardiovascular pelo aumento dos valores de pressão arterial e maior prevalência de demência, 5 vezes mais elevada do que nos indivíduos não alcoólicos. A depressão é uma comorbilidade frequente. Paralelamente, 90% dos idosos tomam medicação diariamente, sendo que muitos fármacos banalmente prescritos têm potencial de interação com o álcool, resultando em prejuízo dos mesmos. Alguns métodos foram recomendados para o rastreio do abuso desta substância, estando validados em Portugal o questionário CAGE e o AUDIT, com o intuito de efetuar uma avaliação e diagnóstico precoces. O tratamento dos idosos apresenta resultados benéficos que se equiparam ao tratamento dos indivíduos jovens, sendo que o mais efetivo resulta da associação do tratamento farmacológico com a intervenção psicológica. Contudo, toda esta análise carece de investigação mais aprofundada dadas as lacunas de informação sobre a epidemiologia, rastreio e intervenção terapêutica nos idosos. The alcoholism among the elderly is a growing problem due to the increasing geriatric population and the need of nursing associated with it. With this review it is intended to provide an overview of alcohol consumption in individuals over 65 years through a literature research performed on PubMed and ClinicalKey database in the library of the Hospital of the University of Coimbra. Approximately 50% of the elderly population is a daily alcohol consumer, and 1 to 3% of the geriatric population suffers from alcohol use disorder, which is a major cause of physical and psychological morbimortality and social stress. The chronic consumption of alcohol is associated with impairment to various organs and, to a greater extent, in the elderly. They are more vulnerable to the emergence of liver disease, to a higher cardiovascular risk due to the increase of blood pressure and to a greater prevalence of dementia, five times higher than in the non-alcoholic individuals. Depression is a common comorbidity. At the same time, 90% of the elderly take daily medication, whereas many of the prescribed drugs have the potential to interact with alcohol, resulting in prejudice of the same. Some methods were recommended to trace this substance abuse, being CAGE and AUDIT the validated questionnaires in Portugal, in order to make an early assessment and diagnosis. Treatment presents with beneficial results that are equivalent to those of younger subjects. Moreover, the most effective treatment results from the association of the pharmacological treatment with the psychological intervention. However, all this analysis requires further investigation given the information gaps on the epidemiology, screening and therapeutic intervention in the elderly.
