Abstract

O presente trabalho analisa a relação entre a psiquiatria e a cultura, apreendendo alguns dos momentos e dos contributos históricos neste domínio e focando-se, especificamente, no impacto e nas variações que as diferenças culturais são suscetíveis de induzir na natureza e manifestações da doença mental que mais curiosidade tem despertado em psiquiatria, a esquizofrenia. Implementou-se um método de revisão da literatura, recorrendo-se, para tal, a obras clássicas no âmbito da psiquiatria transcultural e da psiquiatria cultural, bem como a artigos científicos e de revisão, uns com enfoques mais diacrónicos e outros com incidência mais recente. Os resultados do trabalho são conclusivos ao apontarem a forma diversa como a esquizofrenia afeta e se manifesta em indivíduos oriundos ou vinculados a diferentes contextos culturais, avaliando, ainda, a forma como a migração, enquanto processo relevante de aculturação e de adaptação a uma nova cultura, influencia a incidência/prevalência e as características e manifestações da esquizofrenia. Este trabalho considera, também, a influência do contexto cultural no curso e no prognóstico da doença. As práticas diagnósticas e terapêuticas, condicionadas pelas variadas manifestações sintomáticas da doença e pelas crenças do doente acerca do agente causal são também abordadas, dada a sua grande variabilidade entre o Ocidente e algumas comunidades orientais. Um diagnóstico e tratamento eficazes da esquizofrenia, assim como das demais doenças mentais, necessitam de ter em conta os indivíduos concretos e as suas realidades socioculturais de inserção e interação. A partir da análise dos resultados patenteados neste trabalho de revisão, são delineadas algumas recomendações no âmbito da necessidade de incorporação da variável cultura na abordagem do doente mental em investigação e em consulta de psiquiatria, para que o diagnóstico e tratamento façam sentido não só para o médico, mas também para qualquer doente que se lhe apresente

This paper examines the relationship between psychiatry and culture, learning some of the moments and historical contributions in this area and focusing, specifically, on the impact and variations that cultural differences are likely to induce in the nature and in the manifestations of the mental disease that has aroused more curiosity in psychiatry, schizophrenia. It was implemented a method of literature review, using up to this end the classics within transcultural psychiatry and cultural psychiatry, as well as scientific articles and reviews, ones with more diachronic approaches and others with more recent incidence. Work results are conclusive to point out the different ways in which schizophrenia affects and is manifested in individuals originating from or linked to different cultural contexts, assessing also the way migration, as an important process of acculturation and adaptation to a new culture, influences the incidence/prevalence and the characteristics and manifestations of schizophrenia. This paper also considers the influence of cultural context in the course and prognosis of the disease. Diagnostic and therapeutic practices, conditioned by different symptomatic manifestations of the disease and the patient’s beliefs about the causal agent are also addressed, given its great variability between the West and some eastern communities. An effective diagnosis and treatment of schizophrenia, as well as of other mental diseases, need to take into account the specific individuals and their socio-cultural realities of insertion and interaction. From the analysis of the results patented in this work of revision, we outline some recommendations on the need to incorporate the variable culture in the mental patient approach in research and in the psychiatric appointment, so that diagnosis and treatment make sense not only to the doctor, but also to any patient who submit to him.
