Abstract

Contexto: Ao longo dos últimos anos, a osteoporose tem vindo a revelar-se um problema de saúde pública de crescente importância, contribuindo para o aumento da prevalência de fraturas na população idosa. O objectivo deste trabalho foi proceder a uma revisão dos estudos publicados sobre a eficácia dos bifosfonatos na redução do risco de fratura osteoporótica, o seu impacto nas situações particulares de reparação de fracturas e pós-artroplastia total e, ainda as complicações do uso dos bifosfonatos com destaque para a osteonecrose dos maxilares e para as fracturas atípicas do fémur. Para isso, realizou-se uma extensa pesquisa bibliográfica em bases de dados da literatura ortopédica abrangendo o período de 2000 a 2013. A bibliografia selecionada encontrava-se escrita em língua portuguesa, inglesa e espanhola. As pesquisas foram realizadas recorrendo às base de dados Pubmed, Science Direct (www.sciencedirect.com) e no Repositório Institucional dos HUC (rihuc.huc.min-saude.pt) com o objectivo de encontrar estudos sobre as indicações e complicações dos bifosfonatos em ortopedia, tendo sido selecionadas 27 fontes bibliográficas A metodologia utilizada permitiu concluir que os bifosfonatos são considerados o tratamento de primeira linha em doentes com osteoporose, com eficácia comprovada na redução do risco de fratura osteoporótica. Os bifosfonatos parecem não interferir na reparação cirúrgica de fraturas. Estes fármacos também podem exercer um papel importante na redução de complicações na sequência de Artroplastia Total da Anca (ATA) e Artroplastia Total do Joelho (ATJ) e na manutenção de um boa fixação da prótese. Context: Over the past few years, osteoporosis has proved to be an increasingly important public health problema, contributing to the increased prevalence of fractures in the elderly population. The objective of this work was to carry out a review of published studies about the effectiveness of bisphosphonates in reducing the risk of osteoporotic fractures, its impacto n the particular situations of repair of fractures and after total arthroplasty, and also the complications of bisphosphonate use with emphasis on osteonecrosis of the jaw and atypical femur fractures For that, it was performed an extensive bibliographic research in some databases of medical literature covering the period from 2000 to 2013. The selected bibliography was written in portuguese, english and spanish. The researches were conducted in the Pubmed database and also in Science Direct (www. Sciencedirect.com), and in the Institutional Repository of the HUC (rihuc.huc.min-saude.pt) in order to find studies about the indications and complications of bisphosphonates in orthopaedics, having been selected 27 bibliographic sources. The methodology used allowed to conclude that bisphosphonates are considered the first-line treatment for patients with osteoporosis, with proven effectiveness in reducing the risk of osteoporotic fractures Bisphosphonates do not seem to interfere with the surgical repair of fractures. This drugs may also have and importante role in the reduction of complications in the sequence of Total Hip Arthroplasty and Total Knee Arthroplasty and in the maintenance of a good fixation of the prosthesis.
