Abstract

O hipotiroidismo é definido como uma diminuição da produção de hormonas tiroideias. Há uma destruição ou perda permanente de tecido tiroideu por processos autoimunes ou por irradiação, no hipotiroidismo primário (HT). No hipotiroidismo secundário, existe uma estimulação insuficiente da tiróide pela TSH (thyroid stimulating hormone). É também conhecido o hipotiroidismo subclínico (HTs) e este caracteriza-se por aumento da TSH circulante e uma T4 e T3 livres normais, com pouca ou nenhuma sintomatologia, podendo evoluir para hipotiroidismo. O objetivo deste estudo foi avaliar o impacto das entidades clínicas previamente citadas, no sistema cardiovascular, qualidade de vida e função cognitiva na população idosa (acima de 65 anos de idade nos países desenvolvidos, e acima dos 60 nos países em desenvolvimento). A pesquisa foi efetuada na base de dados PubMed com os seguintes termos: “hypothyroidism” and “elderly”. Foi também consultado um livro de referência. Os estudos considerados neste artigo revelaram que os efeitos da disfunção tiroideia no sistema cardiovascular nos idosos ainda não estão bem estabelecidos e serão, certamente, muito debatidos no futuro, apesar de ter existido alguma correlação positiva entre HTs e insuficiência cardíaca. Os estudos realizados permitem concluir que os pacientes idosos com hipotiroidismo subclínico são mais suscetíveis a desenvolver disfunção cognitiva precoce.

Hypothyroidism is defined as a decrease in the production of thyroid hormones. There is an ongoing destruction or loss of thyroid tissue by autoimmune processes or by irradiation in primary hypothyroidism. In secondary hypothyroidism, there is insufficient thyroid stimulation by TSH (thyroid stimulating hormone). There is also subclinical hypothyroidism (sHT) and it is characterized by high serum TSH and normal free T4 and T3, with scanty symptoms or none, and it can progress to overt hypothyroidism. The aim of this study was to evaluate the impact of hypothyroidism in the cardiovascular system, quality of life and cognitive function in the elderly (over 65 years old in developed countries, and over 60 in developing countries). The research was conducted in the PubMed database with the following terms: "hypothyroidism" and "elderly". A reference book was also consulted. The studies considered in this article revealed that the effects of thyroid dysfunction in the cardiovascular system on the elderly are not well established and will certainly be hardly debated in the future, despite having been some positive correlation between sHT and heart failure. Studies also suggest that the elderly patients with subclinical hypothyroidism are more likely to develop early cognitive impairment.
