Abstract

As artropatias são as manifestações extra-intestinais mais comuns da Colite Ulcerosa, podendo associar-se a morbilidade significativa. A sua prevalência entre estes doentes varia entre 15,8% e 62%. À semelhança da Colite Ulcerosa, a etiologia destas manifestações será multifactorial e permanece não completamente conhecida. A relação entre intestino, sistema imunitário e articulações em indivíduos geneticamente susceptíveis, parece ser crucial para o desenvolvimento destas artropatias. As manifestações articulares são actualmente englobadas no grupo das espondilartropatias seronegativas e são classificadas em duas formas principais: a artrite periférica e a artrite com atingimento axial. O tratamento consiste basicamente na extrapolação da terapêutica usada em outras formas de artrite, através do recurso ao repouso, fisioterapia, injecções locais de esteróides, analgésicos e outros fármacos. No caso particular da artrite periférica tipo I, a medida terapêutica mais importante consiste no controlo, médico ou cirúrgico, da doença intestinal, uma vez que esta forma de artrite se relaciona com a actividade da Doença Inflamatória Intestinal. Em virtude da morbilidade significativa e acréscimo dos custos assistenciais associados, as manifestações articulares da Colite Ulcerosa representam um desafio para o gastroenterologista, para o reumatologista e, num sentido mais amplo, para o próprio sistema de saúde

Arthropathies are the most common extraintestinal manifestations of ulcerative colitis and can lead to significant morbidity. The estimated prevalence among these patients varies from 15.8% to 62%. Much like ulcerative colitis, the etiology of these manifestations is thought to depend on multiple factors and is still not entirely known. The relationship between the intestine, the immune system and the articulations in genetically susceptible individuals seems to be crucial in the development of these arthropathies. Articular manifestations are currently considered to be seronegative spondyloarthropathies and are classified in two major forms of articular involvement: peripheral and axial arthritis. Similarly to other forms of articular inflammation, the treatment of Ulcerative Colitis related arthritis basically consists of rest, physiotherapy, local injections of steroids, analgesics and other drugs. In the particular case of Type I peripheral arthritis, the most important therapeutic action consists in the medical or surgical control of the intestinal inflammation, because it is related to Inflammatory Bowel Disease activity. The morbidity rates along with high economic costs associated with articular manifestations of ulcerative colitis continue to pose a true challenge to the gastroenterologist, the rheumatologist and in a wider sense, to the healthcare system.
