Comorbilidades na psoríase

Abstract

A psoríase é uma doença inflamatória sistémica crónica e imuno-mediada que tem vindo a ser associada a diversas comorbilidades, como hipertensão arterial, diabetes mellitus, obesidade, dislipidémia e síndrome metabólica. Consequentemente tem sido associada um aumento do risco cardiovascular, nomeadamente para a ocorrência de eventos cardiovasculares major como enfarte agudo do miocárdio e acidente vascular cerebral. Esta associação tem sido relacionada com o estado de inflamação sistémica crónica que caracteriza a doença, no qual tem sido atribuída especial relevância ao TNF-α. Neste sentido, os recentes fármacos biológicos inibidores do TNF-α têm evidenciado eficácia terapêutica na diminuição do risco cardiovascular. Esta revisão pretende abordar as comorbilidades cardiometabólicas associadas à psoríase, o papel da inflamação sistémica crónica na fisiopatologia e a eficácia dos fármacos biológicos na diminuição do risco cardiovascular.

Psoriasis is a chronic systemic inflammatory and immune-mediated disease that has been associated with several comorbidities, such as hypertension, diabetes mellitus, obesity, dyslipidemia and metabolic syndrome. Consequently there has also been associated with increased cardiovascular risk, particularly for the occurrence of major cardiovascular events such as acute myocardial infarction and stroke. This association has been linked with a chronic systemic inflammatory status that characterizes the disease, particularly focused in TNF-α. In this sense, recent biologic anti-TNF-α drugs have shown therapeutic efficacy in reducing cardiovascular risk. This article review the association of cardiometabolic comorbidities and psoriasis, the role of chronic systemic inflammation in the pathophysiology and the impact of biologic drugs in reducing cardiovascular risk.
