Abstract

A alergia alimentar define-se como uma resposta imunológica adversa a um alimento. É considerada um importante problema de saúde pública, principalmente nos países ocidentais, onde a sua incidência está a aumentar. A sua prevalência é difícil de determinar, mas estima-se que afete cerca 1 a 2% da população mundial. A clínica da alergia alimentar é sistémica e muito variada. Do ponto de vista imunopatológico, a maioria das reações são decorrentes de hipersensibilidade mediada por Imunoglobulina E (IgE). Mais raramente, a alergia não é mediada por IgE e a sua imunofisiopatologia é ainda pouco conhecida. São reconhecidas distintas entidades clínicas que afetam o sistema digestivo, como os distúrbios intestinais eosinofílicos (mediados por IgE e por células), a enterocolite e proctocolite induzidas pelas proteínas alimentares e a doença celíaca (que ocorrem por mediação celular). Atualmente, a terapia da alergia alimentar consiste, essencialmente, na evicção do alimento causal e no tratamento farmacológico dos episódios agudos. Neste artigo, será apresentado um resumo sobre o que tem sido publicado relativamente a dados epidemiológicos e imunopatologia da alergia alimentar, manifestações digestivas da alergia alimentar e análise das dificuldades diagnósticas que impõem, assim como as opções terapêuticas disponíveis no tratamento da alergia alimentar, dando especial enfoque aos alergénios de origem vegetal.

Food allergy is defined as an adverse immunologic response to one food. It is considered a major public health problem, especially in western countries, where its incidence is increasing. Its prevalence is difficult to calculate, but is estimated to affect approximately 1-2% of the world population. Allergic reactions represent a large array of systemic symptoms. Most of the allergic reactions are immunoglobulin E (IgE) mediated. Rarely, allergies are non-IgE-mediated, but its imunopathophysiology is still poorly known. Distinct clinical entities affecting the digestive system are recognized, such as the eosinophilic gastroenteropathies (resulting from a combined mechanism of IgE and non-IgE), food protein-induced enterocolitis syndrome, food-induced proctocolitis and celiac disease (cell-mediated). Currently, the management of this disease is based on allergen avoidance and pharmacological treatment of acute symptoms. This article reviews recent published data about epidemiological and immunopathology aspects of food allergy, digestive symptoms and the analysis of diagnostic difficulties underneath and the therapeutic options in food allergy, giving particular attention to plant food allergens.
