Abstract

A trombose da veia porta é um acontecimento com prevalência mal definida que nos doentes cirróticos submetidos a transplante hepático pode originar variadas complicações. O objetivo do estudo é conhecer a sua prevalência em cirróticos com indicação para transplante e o impacto que a sua existência possa ter no curso subsequente dos doentes transplantados. Através de dados obtidos nos processos clínicos, foram selecionados todos os doentes com cirrose hepática e posteriormente os que tinham trombose da veia porta entre Março de 2012 e Abril de 2014. Obteve-se uma prevalência de 14,67% (11/75), com grande percentagem de complicações após o transplante (82%) e mortalidade de 27%. Verificaram-se diferenças estatisticamente significativas no tempo de isquemia fria entre os dois grupos. Não se observaram diferenças estatisticamente significativas entre a idade, MELD, tempo de duração da cirurgia e mortalidade. Sabe-se que a anticoagulação e o TIPS (transjugular intrahepatic portosystemic shunt) são duas possíveis abordagens para recanalizar a veia porta.

Portal vein thrombosis is an event with a poorly defined prevalence, which in cirrhotic patients undergoing hepatic transplantation can originate various complications. The purpose of this study is to know its prevalence in cirrhotic patients with indication for transplant and the impact that its existence might have in the subsequent course of transplant patients. Through data obtained in clinical files, there were selected all patients with hepatic cirrhosis and later those who had the portal vein thrombosis between March 2012 and April 2014. It was obtained a prevalence of 14.67% (11/75), with large percentage of complications after transplantation (82%) and 27% mortality. There were statistically significant differences in cold ischemia time among the two groups. No statistically significant differences were observed between age, MELD, duration of surgery and mortality. It is known that anticoagulation and TIPS (transjugular intrahepatic portosystemic shunt) are two possible approaches to recanalize the portal vein.
