Abstract

Objetivo: A asma é a doença crónica mais prevalente em idade pediátrica. Alguns estudos têm sugerido uma relação entre asma e prematuridade, tendo em conta o grau da prematuridade, embora as causas desta associação ainda estejam em investigação. O aumento da prevalência e sobrevida dos prematuros traz o desafio de lidar com um amplo espectro de crianças com asma, o que tem interessado a comunidade científica. Este artigo tem como objetivo investigar a associação entre a prematuridade e a asma em idade pediátrica, de acordo com o grau de prematuridade, em comparação com crianças nascidas a termo. Métodos: Com as palavras-chave preterm, premature birth, prematurity, asthma, children, lung development, foi realizada pesquisa no Pubmed de publicações epidemiológicas analíticas em seres humanos, publicados em inglês entre 2004 e 2014. Publicações adicionais foram identificadas por triagem das listas de referências de artigos de interesse. Foram selecionados os estudos que abordassem a associação entre prematuridade e asma em idade pediátrica. Resultados: Os bebés prematuros têm um risco aumentado de asma na infância em comparação com bebés nascidos a termo. Esse risco é maior à medida que o grau de prematuridade aumenta, sendo a prematuridade extrema (inferior a 32 semanas) a mais afetada. Também foi observado um maior risco de asma para as crianças nascidas antes das 39 semanas. Conclusões: Com a diminuição da idade gestacional, o risco de asma aumenta na infância e idade adulta, estando esta associação na dependência de fatores genéticos, ambientais e perinatais.

Objective: Asthma is the most prevalent chronic disease in children. Some studies have suggested an association between asthma and prematurity, taking into account the degree of prematurity, although the causes of this association are still under investigation. The increasing prevalence and survival of preterm infants challenges to deal with a wide spectrum of children with asthma, which has interested the scientific community. This article aims to investigate the association between prematurity and asthma in children, according to the degree of prematurity, compared with children born at term. Methods: With the keywords preterm, premature birth, prematurity, asthma, children, lung development, research was conducted in Pubmed and were selected analytical epidemiological publications in humans, published in english between 2004 and 2014. Additional publications were identified by screening the articles of interest references. Studies that related the association between preterm birth and asthma in children were selected. Results: Prematurely born children are at increased risk of asthma compared with babies born at term. This risk is greater as the degree of prematurity increases, extremely preterm (less than 32 weeks) is the most affected. There was also an increased risk of asthma in children born before 39 weeks. Conclusions: Decreasing gestational age increases the risk of asthma in childhood and adulthood. This association depends on genetic, environmental and perinatal factors.
