Abstract

O acidente vascular cerebral (AVC) afeta principalmente os idosos, sendo que a depressão é uma das complicações neuropsiquiátricas mais comuns na população no pós-AVC, variando entre 30 a 50% dos doentes durante o primeiro ano. A depressão pós-AVC (DPA) associa-se a uma limitação das atividades da vida diária, a um prejuízo cognitivo, a um risco elevado de recorrência do AVC, e um pobre resultado de reabilitação, levando assim a piores evoluções do quadro clínico, pior aderência terapêutica e pior qualidade de vida com maior morbilidade e mortalidade. Foi realizado uma revisão da literatura disponível sobre a DPA no idoso, analisando estudos e revisões da bibliografia. Foi usado o PUBMED, uma revisão de dois relatórios da OMS, uma revisão e uma orientação da Direção Geral de Saúde assim como o Diagnostic and Statistical Manual of mental disorders – fifth edition. Verificou-se uma correlação entre a DPA e os idosos, apesar desta ser mais prevalente nos mais novos. Também verificou-se que as mulheres mais velhas relacionavam-se com a DPA, enquanto os homens apresentavam maior risco em idades mais jovens. De uma maneira geral existe uma dependência encontrada entre a DPA, o défice cognitivo e o prejuízo funcional. A DPA também leva a pobre qualidade de vida, assim como aumenta a mortalidade e sendo necessário um tratamento para prevenir esta entidade. O objetivo do trabalho pretende fazer uma revisão da literatura disponível da depressão na população idosa após um AVC, mais particularmente na sua incidência, nos fatores de risco e no seu impacto na qualidade de vida do doente.

Stroke principally appears on old people and depression is one of the most common neuropsychiatric complication in post-stroke population, which has a variation from 30 to 50% during the first year. Post-stroke depression is associated with limitation of day life activities, cognitive injury, high risk for stroke recurrence and poor rehabilitation effects, resulting in worse clinical evolutions, poorer therapeutic adherence, worse life quality and greater morbidity and mortality. I did a background of the available literature about post-stroke depression in the elderly, analyzing studies and reviews. I also used Pubmed, two Mundial Health Organization studies, a General Health Direction Study (Portugal) and Diagnostic and Statistical Manual of Mental Disorders– fifth edition. I noticed a correlation between post-stroke depression and old people, despite depression being more prevalent in younger people. Similarly, I observed a correlation with post-stroke depression and women, but men have more risk in younger ages. Generally, there is dependence between post-stroke depression and cognitive and functional injuries. Post-stroke depression improves poor life quality, as well as mortality, being necessary to be treated to prevent. The aim of this study is to look for the available literature of post-stroke depression in the elderly but specially focus on the incidence, risk factors and on its impact on life quality.
