Abstract

Contexto: A presente dissertação de Mestrado desenvolvida no âmbito do Mestrado em Saúde Pública, pretende desenvolver o tema “Cefaleias como reação adversa a fármacos” de forma a identificar estado da arte da notificação de cefaleias provocadas por fármacos. Objetivos: Identificação de notificação de cefaleias como reação adversa a fármacos. Métodos: De forma a concretizar o objetivo definido, foi efetuada uma pesquisa na base de dados da Unidade de Farmacovigilância do Centro (UFC) de modo a identificar as notificações que contivessem o termo “headaches”, registadas durante o período de 2001 até ao presente. Resultados: No período referido, existe um total de 2662 notificações, em que 133 dessas notificações contêm o termo “headaches”. Cerca de 79% dos casos dizem respeito ao sexo feminino e cerca de 21% dizem respeito ao sexo masculino. Sendo também possível identificar diferenças entre idades; de acordo com a informação recolhida da UFC é possível identificar cinco intervalos de idade, nos quais as cefaleias são mais frequentes, nomeadamente, dos [30;40[, dos [40;50[, dos [50;60[, dos [60;70[ e dos [70;80[ anos. Ao longo do tempo é possível verificar uma tendência para a diminuição das notificações de cefaleias, assim com uma tendência decrescente de RAM’s graves. Quanto ao nexo de causalidade, 12% das reações adversas medicamentosas (RAM’s) foram classificadas como definitivas, 14% RAM’s como prováveis, 19% como possíveis, 2% como Improváveis, 1% como Condicionais, 5% como não Classificáveis e em 47% não existe informação. A identificação das notificações por grupo terapêutico, do medicamento em causa, é também um dos focos mais importantes, sendo o grupo 2. Sistema Nervoso Central e o grupo 3. Aparelho Cardiovascular, os grupos nos quais se verifica uma maior percentagem de notificações. Dentro do grupo 3. Aparelho Cardiovascular, é possível identificar que dos fármacos em causa 21% correspondem a fármacos vasodilatadores. Conclusões: A notificação de cefaleias na UFC é reduzida, provavelmente por fatores como a elevada prevalência de cefaleias na população em geral, as cefaleias serem consideradas fenómenos de baixa relevância, elevados níveis de polimedicação. Todos estes fatores resultam numa elevada dificuldade de atribuição do nexo de causalidade com o medicamento em causa. Deste modo, torna-se fundamental a nível de promoção da saúde da população a promoção de campanhas de chamada de atenção ao tema, dinamização de sessões de esclarecimento e educação da comunidade numa maior preocupação e atenção para as cefaleias.

Context: This Master Thesis had been developed to obtain the degree of Master in Public Health. The theme developed was "Headache as an adverse reaction to drugs" in order to identify the art of headache notification as an adverse reaction to drugs. Objective: Identification of headache notification as an adverse reaction to drugs. Methods: In order to achieve our goal, a research was performed in the database of the Pharmacovigilance Unit of the Centre Region (UFC) to identify the notifications containing the term "headaches", documented from 2001 to present time. Results: For the period delivered there was a total of 2662 notifications, from this, 133 notifications contained the term "headaches". Checking the difference between genders, about 79% of cases concern women and about 21% were related to men. As well as gender differences, was also identifiable differences between ages, according to UFC information was possible to identify five age intervals in which headaches are more common , in particular, [30, 40[, [40; 50[, [50, 60[, [60, 70[ and [70, 80[ years. Over time, it is possible to observe a decrease tendency of headache notifications as well as a decrease on its severity. In relation to the causal link, 12% of the adverse drug reactions (ADR’s) are classified as definitive, 14% of ADR’s as probable, 19% as possible, 2% as improbable, 1% as conditional and 5% as without information. The identification of notifications by therapeutic group of the concerned medicinal product is also one of the most important topics; Group 2. Central Nervous System and group 3. Cardiovascular System, are the ones with a larger percentage of notifications. In group 3. Cardiovascular System, 21% of the total notifications are related to vasodilators. Conclusions: Nowadays, the notification of headaches is reduced, probably by factors such as the high prevalence of headaches in the general population, being considered as a low relevance phenomen and high levels of polymedication. All these factors result in a high difficulty to allocate a causal link with the medicinal product. Consequently, in Public Health is very important the promotion of warning actions, information sessions and community education to increase the concerns and attention to headaches.
