Abstract

Introdução: A endocardite infecciosa é uma doença multifactorial, que sofreu mudanças epidemiológicas nas últimas décadas, sendo encarada como um desafio diagnóstico. Objectivos: O objectivo principal do estudo foi a análise da taxa de mortalidade intra-hospitalar e identificação de factores preditores/protectores da mesma. Métodos: Análise retrospectiva a 78 processos clínicos consecutivos de pacientes com diagnóstico principal e secundário de endocardite infecciosa, admitidos no Serviço de Cardiologia dos Hospitais Universidade de Coimbra, entre 2005 e 2010. Resultados: A idade média da população era 60,7 (46,3 – 75,1) anos, com predominância do sexo masculino (75,6%). A mortalidade hospitalar foi de 37,2%. Esta encontrava-se associada com o aumento da idade, diabetes mellitus, neoplasias, valor máximo dos parâmetros inflamatórios e intervenção cirúrgica. Nenhuma outra relação foi identificada entre os antecedentes do doente, critérios de Duke, diagnóstico, apresentação clínica, dados ecocardiográficos e a mortalidade intra-hospitalar. Na análise multivariada, apenas a idade e a referenciação para cirurgia foram preditores independentes do evento referido. Conclusões: A mortalidade intra-hospitalar foi de 37,2%; a idade foi o único factor de risco relativo para a ocorrência de morte intra-hospitalar, sendo a cirurgia um factor protector

Introduction: Infective endocarditis is a multifactorial disease, which experienced epidemiological changes in the last decades and it is considered as a diagnostic challenge. Objectives: The primary objective was to determinate the rate and to identify independent predictors of the event. Methods: Retrospective study of 78 patients with a principal or secondary diagnosis of infective endocarditis admitted to the Cardiology Department of the Hospitais da Universidade de Coimbra, between 2005 and 2010. Results: Mean age of the population was 60,7 (46.3 – 75.1) with a male predominance (75.6%). The in-hospital mortality was 37.2%. It was associated with increasing age, diabetes, cancer, peak value of inflammatory markers during admission and in hospital surgery. No relation was identified regarding other medical history variables, Duke diagnostic score, clinical presentation, and echocardiographic data and in hospital mortality. On a multivariate model, only age and in hospital surgery were independent predictors of the refered event. Conclusions: In our population of infectious endocarditis, age and in hospital surgery independently predicted in hospital mortality.
