Abstract

Introdução: A lombalgia assume-se actualmente como uma das principais causas de dor crónica, afectando uma percentagem significativa da população. É uma causa importante de incapacidade com repercussões a nível pessoal, sócio-económico e profissional. Diversos factores associados ao estilo de vida estão relacionados com este sintoma, contribuindo para a deterioração da qualidade de vida nestes doentes. Objectivo: Avaliar o impacto da dor lombar crónica na qualidade de vida dos doentes, tendo em conta a função, a interacção social, profissional e o estado de saúde. Metodologia: Estudo clínico, não randomizado, realizado na região centro de Portugal, incluindo 34 doentes com dor lombar crónica, de ambos os sexos, entre os 45 e os 65 anos de idade. Foram utilizados os seguintes instrumentos métricos validados em língua portuguesa: Oswestry Disability Index (ODI) – versão 2.0, Short-form do International Physical Activity Questionnary (IPAQ) e Medical Outcomes Study – Short Form 36 itens (SF-36). A análise estatística da amostra foi efectuada em SPSS 17.0 (Statistical Package for Social Sciences) para o Windows. Resultados: A amostra estudada compreendia maioritariamente doentes do sexo feminino (76,5%) e a duração média da dor era de 15,49 anos. Os resultados obtidos nos doentes com lombalgia apresentam uma diferença estatisticamente significativa em todas as dimensões do SF-36 em relação a uma população portuguesa saudável. Verificou-se uma relação inversa entre o grau de incapacidade medido pelo ODI e alguns parâmetros de estado de saúde do SF-36 dos doentes. Aqueles que consomem menos fármacos anti-depressivos e ansiolíticos (AD/AS) apresentam melhores resultados ao nível do Desempenho Físico, Função Social e Saúde Mental do SF-36. Conclusões: Os doentes com dor lombar crónica apresentam um grau de incapacidade significativa com deterioração multidimensional da qualidade de vida. Background: Low back pain is currently assumed as one of the main causes of chronic pain, affecting a significant proportion of the population. It’s a major cause of disability with impact at personal, socio-economic and professional level. Several factors related to “life style” are associated with deterioration of the quality of life in these patients. Objective: To evaluate the impact of chronic low back pain on patients’ quality of life, taking into account the function, social interaction and state of health. Methods: Clinical study, not randomized, performed in the central region of Portugal, including 34 patients with chronic low back pain, of both sexes, between the 45 and the 65 years-old. Were used the following metric instruments validated in Portuguese: Oswestry Disability Index (ODI) – version 2.0, Short-form of International Physical Activity Questionnary (IPAQ) e Medical Outcomes Study – Short Form 36 items (SF-36). The statistical analysis of the sample was performed in SPSS 17.0 (Statistical Package for Social Sciences) for Windows. Results: The studied sample included mostly female patients (76,5%) and the average time of pain was 15,49 years. The results in patients with low back pain showed a relevant statistical difference in all dimensions of the SF-36 when compared with a healthy Portuguese population. It was found an inverse correlation between the degree of disability measured by ODI and some of the parameters of health of the SF-36. Those who consume less anti-depressives and anxiolytics (AD/AS) present best results at Physical Performance, Social Function and Mental Health of the SF-36. Conclusions: The patients with chronic low back pain present a significant degree of disability with multidimensional deterioration of quality of life.
