Abstract

IntroduçãoA PF visa garantir a capacidade reprodutiva de pacientes através do armazenamento de gâmetas ou tecidos reprodutivos. Dado que o Serviço de Medicina da Reprodução pertencente à ULS Coimbra, é atualmente o único centro de referência para PF em Portugal, a realização deste estudo é essencial para fornecer uma visão abrangente do estado atual da PF em pediatria. Com esse intuito, este trabalho tem como objetivo principal estudar a PF em crianças e adolescentes em Portugal, caracterizando demograficamente os pacientes, as principais condições ou patologias que os colocam em risco de infertilidade e as opções disponíveis para preservar a sua fertilidade.Materiais e MétodosForam consideradas todas as crianças observadas nas consultas de PF no Serviço de Medicina da Reprodução da ULS Coimbra no período de 1 de janeiro de 2012 a 31 de dezembro de 2024.O co-investigador pertencente ao Serviço de Medicina da Reprodução da ULS Coimbra, procedeu à colheita da informação mínima indispensável para a realização da investigação a partir da consulta do registo clínico da plataforma do Serviço. A pesquisa dos doentes foi realizada pela idade do doente à data de observação em consulta. Os dados foramanalisados nas plataformas Excel e SPSS v29, onde se realizaram medidas de estatística descritiva.ResultadosEste estudo incluiu 70 utentes que recorreram à consulta de Medicina da Reprodução para PF, com idades entre 0 e 17 anos. As condições médicas que motivaram a consulta foram principalmente neoplasias (77,1%), com destaque para o Linfoma de Hodgkin. A maioria dos utentes (81,4%) prosseguiu com a PF, sendo que no sexo feminino predominou a ooforectomia com criopreservação de tecido ovárico (78%) e no sexo masculino a criopreservação de esperma (56%). A preservação foi realizada antes do início de tratamentos oncológicos em 56,1% dos casos. Não foram encontradas diferenças estatisticamente significativas entre os grupos que preservaram e os que não preservaram em relação à idade e às condições clínicas que motivaram a consulta.Discussão e ConclusãoAnualmente são diagnosticados em média 400 casos de cancro entre crianças e adolescentes em Portugal. Nos 12 anos do estudo foram referenciados apenas 70 utentes em idade pediátrica para a consulta de PF. Este estudo revelou que a idade e as condições clínicas não foram fatores decisivos para a escolha da preservação. A análise destaca a importância de uma abordagem precoce e integrada, ressaltando a relevância da PF como parte essencial do tratamento e da garantia da qualidade de vida futura dos pacientes.

IntroductionFertility preservation (FP) aims to ensure the reproductive capacity of patients through the storage of gametes or reproductive tissues. Since the Reproductive Medicine Service of ULS Coimbra is currently the only reference center for FP in Portugal, conducting this study is essential to provide a comprehensive overview of the current state of FP in pediatrics. With this purpose, the main objective of this study is to examine FP in children and adolescents in Portugal, characterizing the patients demographically, identifying the main conditions or pathologies that put them at risk of infertility, and exploring the available options to preserve their fertility.Materials and MethodsAll children seen in FP consultations at the Reproductive Medicine Service of ULS Coimbra between January 1, 2012, and December 31, 2024, were considered.The co-investigator, affiliated with the Reproductive Medicine Service of ULS Coimbra, collected the minimum necessary information for the study from clinical records on the Service's platform. Patient searches were conducted based on the age of the patient at the time of consultation. Data analysis was performed using Excel and SPSS v29, applying descriptive statistical measures.ResultsThis study included 70 patients who attended the Reproductive Medicine consultation for FP, aged between 0 and 17 years. The primary medical condition leading to consultation was neoplasms (77.1%), with Hodgkin’s lymphoma being the most prominent. The majority of patients (81.4%) proceeded with FP, with ovarian tissue cryopreservation via oophorectomy being the predominant method in females (78%) and sperm cryopreservation in males (56%). Preservation was performed before the initiation of oncological treatments in 56.1% of cases. No statistically significant differences were found between the groups that preserved fertility and those that did not regarding age or the medical conditions leading to consultation.Discussion and ConclusionOn average, 400 cases of cancer are diagnosed annually among children and adolescents in Portugal. Over the 12 years of the study, only 70 pediatric patients were referred for FP consultation. This study revealed that age and clinical conditions were not decisive factors in the choice of preservation. The analysis highlights the importance of an early and integrated approach, emphasizing the relevance of FP as an essential part of treatment and a key factor in ensuring patients' future quality of life.
