Abstract

Resumo:O papiloma invertido é um tumor histologicamente benigno. Não obstante, apresenta elevada taxa de recorrência, agressividade local e potencial de transformação maligna, principalmente em carcinoma de células escamosas. A localização no seio esfenoidal, além de rara, está associada a uma maior taxa de recorrência e de complicações pelas estruturas vitais que lhe são adjacentes.Relata-se o caso clínico de um doente com sintomas de obstrução nasal bilateral, secreções nasais sanguinolentas e cefaleia occipital com anos de evolução. O exame objetivo e os exames complementares de diagnóstico, tomografia computadorizada e ressonância magnética dos seios paranasais, sugeriram a hipótese de papiloma invertido do seio esfenoidal. Para efeitos de diagnóstico histológico e tratamento, o doente foi proposto para biópsia excisional endoscópica. Durante a cirurgia, foi revelada uma área de exposição da dura-máter na parede posterior do seio esfenoidal e inserção do tumor na região da impressão carotídea esquerda. A lesão foi removida na totalidade e sem complicações. A análise histopatológica confirmou a hipótese de papiloma invertido. O doente não apresenta evidência clínica de recidiva até ao momento.O objetivo deste relato é sinalizar a importância de um adequado acompanhamento, demonstrando que mesmo perante sintomas crónicos e inespecíficos se devem considerar diagnósticos mais raros. O tratamento precoce facilita o gesto cirúrgico e pode evitar sérias complicações.Palavras-chave: papiloma invertido, seio esfenoidal, tumores dos seios paranasais, desmineralização patológica óssea, dura-máter, cirurgia endoscópica por orifício natural

Abstract:Inverted papilloma is a histologically benign tumor. Nonetheless, it has a high rate of recurrence, local aggressiveness, and the potential for malignant transformation, especially into squamous cell carcinoma. Location in the sphenoid sinus, although rare, is associated with a higher recurrence rate and complications due to the adjacent vital structures.We report the clinical case of a patient with symptoms of bilateral nasal obstruction, bloody nasal secretions, and occipital headache over several years. The physical examination and complimentary diagnostic tests, computed tomography and magnetic resonance imaging of the paranasal sinuses, suggested the hypothesis of an inverted papilloma of the sphenoid sinus. The patient was referred for endoscopic excisional biopsy for histologic diagnosis and treatment. During surgery, an area of exposure of the dura mater on the posterior wall of the sphenoid sinus and an insertion of the tumor in the region of the left carotid indentation were revealed. The lesion was completely excised without complications. Histopathologic analysis confirmed the hypothesis of inverted papilloma. The patient has had no recurrence up to date.The aim of this report is to emphasize the importance of proper follow-up and to illustrate that even in the case of chronic, non-specific symptoms, rare diagnoses should be considered.
