Neuroendoscopia no tratamento da hidrocefalia - casuística e análise morfométrica dos ventrículos cerebrais

Abstract

Introdução: A hidrocefalia é uma condição neurológica caracterizada pela acumulação anormal de líquido cefalorraquidiano (LCR), frequentemente tratada com intervenção cirúrgica. Tem uma prevalência global de 85 por 100.000. As técnicas neuroendoscópicas tornaram-se o gold standard para diversos procedimentos neurocirúrgicos, especialmente para patologias intraventriculares como a hidrocefalia, mas a definição de sucesso terapêutico continua ambígua.Métodos: Realizou-se um estudo retrospetivo, descritivo e observacional com doentes adultos tratados com técnicas neuroendoscópicas no contexto de hidrocefalia, entre 2013 e 2024, na ULS Coimbra. Foram recolhidos dados clínicos, imagiológicos e morfométricos. Os doentes foram divididos em dois grupos segundo a etiologia: crónica e pós-traumática. A eficácia foi avaliada com base nas melhorias clínicas e nas alterações nos parâmetros ventriculares.Resultados: A ventriculostomia endoscópica do terceiro ventrículo (ETV) foi realizada isoladamente em 73,3% dos doentes. Casos combinados incluíram septostomia, derivação ventriculoperitoneal (DVP) e fenestração de quisto. Observou-se melhora clínica significativa, com redução da incontinência urinária (72,7% para 25%), alterações da marcha (81,8% para 25%) e do equilíbrio (63,6% para 25%). Etiologias crónicas (estenose do aqueduto, quisto intraventricular e hidrocefalia de pressão normal [HPN]) mostraram melhor evolução e menor necessidade de reintervenção. Houve redução inicial dos volumes ventriculares e do índice de Evans, com discreta reexpansão no follow-up. Comparando pré-intervenção e no follow-up, as reduções foram de 15,85% nos ventrículos laterais, 36,27% no terceiro ventrículo e 7,54% no índice de Evans. Nos doentes com traumatismo cranioencefálico (TCE), 75% precisaram de reintervenção e houve dois óbitos.Discussão: As técnicas neuroendoscópicas revelaram-se eficazes na maioria dos casos, especialmente na hidrocefalia obstrutiva. Os resultados demonstram que a melhoria clínica nem sempre se correlaciona com as alterações imagiológicas.Conclusão: A neuroendoscopia é uma abordagem terapêutica segura e eficaz na hidrocefalia obstrutiva em adultos. Estudos prospetivos mais amplos são necessários para confirmar os achados e definir critérios de sucesso clínico e imagiológico.

Introduction: Hydrocephalus is a neurological condition characterized by abnormal accumulation of cerebrospinal fluid (CSF), often treated with surgical intervention. It has a global prevalence of 85 per 100,000. Neuroendoscopic techniques have become the gold standard for various neurosurgical procedures, particularly for intraventricular pathologies like hydrocephalus, but the definition of therapeutic success remains ambiguous.Methods: A retrospective, observational, and descriptive study was conducted on adult patients treated with neuroendoscopic techniques for hydrocephalus, between 2013 and 2024, at ULS Coimbra. Clinical, imaging, and morphometric data were collected. Patients were grouped in chronic and post-traumatic etiology. Clinical improvement and ventricular volume changes were used to assess effectiveness.Results: Endoscopic Third Ventriculostomy (ETV) was performed as a standalone procedure in 73.3% of patients. Combined cases included septostomy, ventriculoperitoneal shunt, and cyst fenestration. Significant clinical improvement was observed, with reductions in urinary incontinence (from 72.7% to 25%), gait disturbances (81.8% to 25%), and balance issues (63.6% to 25%). Chronic etiologies (aqueductal stenosis, intraventricular cyst, and normal pressure hydrocephalus [NPH]) showed better clinical outcomes and lower reintervention rates. An initial reduction in ventricular volumes and Evans index was noted, followed by slight re-expansion during follow-up. Comparing pre-intervention and follow-up values, reductions were 15.85% in lateral ventricles, 36.27% in the third ventricle, and 7.54% in the Evans index. In the traumatic brain injury (TBI) group, 75% required reintervention, and there were two deaths.Discussion: Neuroendoscopic techniques proved effective, especially in obstructive hydrocephalus. Clinical improvement did not always match radiological changes.Conclusion: Neuroendoscopy is a safe and effective treatment for obstructive hydrocephalus in adults. Larger prospective studies are needed to validate these findings and define success criteria.
