Abstract

A tuba auditiva (TA) desempenha um papel essencial na ventilação do ouvido médio, na proteção contra microorganismos da nasofaringe e na drenagem de secreções do ouvido médio. A disfunção da tuba auditiva (DTA), especialmente a disfunção obstrutiva, compromete estas funções, resultando em sintomas como otalgia, plenitude auricular, zumbidos e perda auditiva. O tratamento da DTA tem tradicionalmente envolvido abordagens médicas, como descongestionantes nasais, antihistamínicos e corticosteroides tópicos. Contudo, uma proporção significativa de doentes mantém sintomas refratários, o que tem impulsionado a procura de outras abordagens mais eficazes. A dilatação da tuba auditiva por balão (DTAB), introduzida em 2010, surge como uma opção minimamente invasiva que visa restaurar a permeabilidade da TA através da aplicação de pressão sobre a porção cartilaginosa da TA. Desde a sua introdução, vários estudos têm demonstrado melhorias significativas na função da TA, evidenciadas por testes audiológicos, manométricos e pela redução dos sintomas reportados pelos doentes. No entanto, embora os resultados sejam encorajadores, a sua eficácia tem sido questionada. Esta revisão narrativa tem como objetivo explorar a evidência científica disponível sobre a DTAB, abordando sumariamente a técnica cirúrgica e de forma mais detalhada a sua eficácia e segurança, bem como complicações e limitações associadas a este procedimento. Os resultados analisados sugerem que a DTAB proporciona melhorias significativas tanto nos sintomas subjetivos quanto nos parâmetros objetivos, como otoscopia, timpanometria, manobra de Valsalva e tubomanometria. Além disso, as complicações são raras e quando surgem, tendem a ser ligeiras, o que reforça o perfil de segurança do procedimento. Apesar destes resultados promissores e da DTAB representar uma alternativa cirúrgica inovadora, mantem-se a necessidade de desenvolver, futuramente, ensaios clínicos randomizados, prospetivos e com amostras mais representativas, a fim de consolidar o seu papel no algoritmo terapêutico da DTA.

The Eustachian tube (ET) plays a crucial role in middle ear ventilation, protection against microorganisms from the nasopharynx, and draining middle ear secretions. Eustachian tube dysfunction (ETD), particularly obstructive dysfunction, compromises these functions, resulting in symptoms such as otalgia, aural fullness, tinnitus, and hearing loss. The treatment of ETD has traditionally involved medical approaches, including nasal decongestants, antihistamines, and topical corticosteroids. However, a significant proportion of patients experience refractory symptoms, driving the search for more effective approaches.Balloon Eustachian tuboplasty (BET), introduced in 2010, has emerged as a minimally invasive option aimed at restoring ET patency by applying pressure to the cartilaginous portion of the ET. Since its introduction, several studies have demonstrated significant improvements in ET function, evidenced by audiological and manometric tests, as well as the reduction of patient-reported symptoms. Nonetheless, despite encouraging results, its effectiveness has been questioned.This narrative review aims to explore the available scientific evidence on BET, providing a brief overview of the surgical technique and a more detailed analysis of its efficacy and safety, as well as complications and limitations associated with the procedure.The analyzed results suggest that BET provides significant improvements in both subjective symptoms and objective parameters, such as otoscopy, tympanometry, Valsalva maneuver, and tubomanometry. Additionally, complications are rare and, when they do occur, tend to be mild, reinforcing the procedure's safety profile.Despite these promising findings and the fact that BET represents an innovative surgical alternative, there remains a need for future prospective, randomized clinical trials with larger and more representative samples to further establish its role within the ETD treatment algorithm.
