Abstract

Introdução: O colesteatoma é uma neoplasia benigna do ouvido médio, caracterizada pelo seu comportamento expansivo e localmente agressivo, associando-se a um risco significativo de complicações intra e extracranianas. O tratamento é cirúrgico, sendo a mastoidectomia a abordagem de eleição, com o objetivo de erradicar a lesão e preservar, sempre que possível, a função auditiva. No caso clínico em apreço, estamos perante uma apresentação atípica de colesteatoma, destacando-se pela sua apresentação bilateral, com múltiplas cirurgias prévias e reabilitação auditiva, com bom ganho funcional através de aparelhos auditivos convencionais, após mastoidectomia radical bilateral. Caso Clínico: Descrevemos o caso de uma doente do sexo feminino, de 34 anos de idade, com colesteatoma bilateral e antecedente de múltiplas mastoidectomias canal wall-up, referenciada à consulta de Otorrinolaringologia por surdez de transmissão bilateral e otorreia purulenta. Foi submetida a mastoidectomia canal wall-down bilateral com meatoconcoplastia de três retalhos, com evolução pós-operatória favorável. A reabilitação auditiva com aparelhos auditivos convencionais bilaterais permitiu bom ganho funcional em ambos os ouvidos. Conclusão: Tendo em consideração a natureza expansiva e comportamento localmente agressivo do colesteatoma, bem como a complexidade associada à sua abordagem terapêutica, sublinha-se a importância de um diagnóstico precoce, um planeamento cirúrgico adequado e uma reabilitação auditiva personalizada de forma a maximizar os resultados funcionais e qualidade de vida do doente. Associadamente, salienta-se a relevância de um seguimento contínuo e da educação do doente relativa a sinais de recorrência, garantindo um acompanhamento preventivo e eficaz.

Introduction: Cholesteatoma is a benign neoplasm of the middle ear, characterized by its expansive and locally aggressive behavior, which entails a significant risk of intra and extracranial complications. Surgical intervention is the treatment of choice, with mastoidectomy being the preferred approach, aiming to eradicate the lesion while preserving auditory function whenever feasible. This case report presents an atypical manifestation of cholesteatoma, distinguished by its bilateral occurrence, a history of multiple previous surgical interventions, and successful auditory rehabilitation with conventional hearing aids, achieving favorable functional outcomes following radical mastoidectomy. Case Report: We describe the case of a 34-year-old female patient with bilateral cholesteatoma and a history of multiple canal wall-up mastoidectomies, referred to the Otorhinolaryngology clinic due to moderate bilateral conductive hearing loss and purulent otorrhea. She underwent bilateral canal wall-down radical mastoidectomy with three-flap meatoconchoplasty, with a favorable postoperative course. Auditory rehabilitation with conventional bilateral hearing aids resulted in satisfactory functional improvement in both ears. Conclusion: Given the expansive and locally aggressive nature of cholesteatoma, as well as the complexity associated with its management, this case underscores the critical importance of early diagnosis, meticulous surgical planning, and tailored auditory rehabilitation to optimize functional outcomes and enhance patients’ quality of life. Moreover, the necessity of continuous follow-up and patient education regarding signs of recurrence is emphasized to ensure preventive and effective long-term management.
