Linfoma primário da coluna vertebral: caso clínico

Abstract

Os linfomas ósseos são pouco frequentes e raramente estão localizados à coluna vertebral, constituindo estas localizações geralmente manifestações secundárias. Os autores apresentam o caso clínico de uma doente de 58 anos de idade, com lombalgia mecânica de 6 semanas de evolução, sem resposta a terapêutica analgésica e anti-inflamatória e sem outras queixas acompanhantes. O estudo complementar revelou a existência de anemia normocítica, VS elevada, radiografia da coluna lombar com discreto achatamento vertebral de L2 e hiperfixação a esse nível no cintigrama osteo- articular. A TAC e a RMN mostraram lesões osteolíticas de D12, L1 e L2 e o estudo histológico da biopsia de L2 revelou linfoma não Hodgkin. A doente foi submetida a cirurgia de estabilização, quimioterapia CHOP e radioterapia local. Cumprido o plano terapêutico previsto, a doente manteve dor localizada; alguns meses depois, surgiram adenomegalias cervicais e a doente faleceu cerca de um ano após o diagnóstico.

Lymphomas of the bone are uncommon and only occasionally encountered in the spine, where they are usually secondary deposits. The authors report the case of a fifty-eightyear old woman with low back pain of six weeks duration, which did not improve with analgesics. Laboratory tests showed a normocytic anaemia and a raised erythrocyte sedimentation rate. Spinal X-Rays showed collapse of the L2 vertebra and increased uptake at this level in the bone scan. The CT and the MRI demonstrated lytic bone lesions in D12, L1 e L2 vertebrae and the histological examination of the biopsy specimen revealed a Non-Hodgkin Lymphoma. The patient was managed with surgery to stabilise the vertebra, chemotherapy with the CHOP regime and local irradiation. After therapy, the patient maintained back pain, cervical lymph nodes appeared and the patient died a year after the diagnosis.
