Introduction: IgA Nephropathy (IgAN) is the most common primary nephropathy in the world, characterized by the mesangial deposition of immune complexes containing galactose deficient immunoglobulin A. It has particular incidence among young adults, with its risk of progression to end-stage kidney disease being recognized in up to 40% of all patients in 20 to 30 years after its diagnosis, demonstrating this nephropathy’s clinical and socioeconomic impact. Increased knowledge regarding its pathogenesis allowed for the identification of specific therapeutic targets, placing IgAN’s approach in a particularly exciting new era of therapeutic investigation. Therefore, it becomes relevant to do a global revision of IgAN’s existing literature, having as goal the comprehension of the biggest proposals and discoveries regarding IgAN’s main pathogenic mechanisms and therapeutic approaches.Methods: The Pubmed and Scopus databases were consulted, limiting the results to articles published between january of 2014 and march of 2024. Among the obtained results, a selection was made having into consideration the pertinence and relevance of each article to the scientific community. The 2022 report of the Office for the National Registry of Kidney Biopsies (Gabinete de Registo Nacional de Biópsias Renais) belonging to the Portuguese Society of Nephrology (Sociedade Portuguesa de Nefrologia), was also consulted.Discussion: The multi-hit model remains as the consensual explanation regarding pathogenesis, however, genetic factors, the proposed interaction of a mucosa-kidney axis and the activation of both the lectin and the alternative pathways of the complement system are factors whose involvement is considered crucial towards the induction of kidney injury. There are no validated specific disease biomarkers and kidney biopsy remains the sole method of establishing definitive diagnosis, allowing for the finding of IgA as the dominant or codominant immunoglobulin in mesangial deposits, in immunofluorescence microscopy. Those deposits produce variable clinical responses, however, the most common clinical presentation is that of microscopic hematuria associated with variable degrees of proteinuria and progressive chronic kidney disease. Oxford’s MEST-C classification and tools such as IIgAN-PT are key to predict prognosis, but still unfit to guide treatment. Supportive treatment remains as the cornerstone of clinical management for all patients, with the purpose of delaying the progression of chronic kidney disease. For patients exhibiting high risk of progression, a cycle of systemic immunosuppression (systemic corticosteroid with or without the addition of mycophenolate mofetil) is recommended, with considerable side effects, having been approved an alternative therapeutic approach consisting of an enteric-release budesonide. In patients not subject or susceptible to immunosupression, that mantain persistent proteinuria despite optimized supportive treatment with an angiotensin-converting enzyme inhibitor or an angiotensin II receptor angatonist, novel therapeutic approaches were approved, such as the association of an SGLT2 inhibitor or replacement with Sparsentan. Novel agents, such as complement system modulators and agents targeting B lymphocytes, are undergoing clinical trials with promising results.Conclusions: The novel therapeutic approaches in study, based on the greater understanding regarding IgAN’s pathogenesis, lead to the belief that the future will bring about the existence of valid alternatives, with effective strategies approaching different pathogenic elements of this chronic immunological disease, in an invidualized fashion. This individualization is the greatest challenge in IgAN patients’ therapeutic approach while we await for the discovery of specific disease biomarkers able to guide therapeutic decisions in a way that is trustworthy, safe and reproducible.
Introdução: A nefropatia por IgA (IgAN) é a nefropatia primária mais comum do mundo, caracterizando-se pela deposição mesangial de imunocomplexos contendo imunoglobulina A deficiente em galactose. Incide, particularmente, em jovens adultos, sendo reconhecido o seu risco de progressão para doença renal terminal em até 40% dos doentes ao fim de 20-30 anos do diagnóstico, demonstrando o impacto clínico e socioeconómico desta nefropatia. O entendimento crescente da sua patogénese permitiu identificar novos alvos terapêuticos específicos, colocando a abordagem da IgAN numa nova era de investigação terapêutica particularmente entusiasmante. Torna-se, assim, relevante a revisão global da literatura existente sobre a IgAN, tendo como objetivo a compreensão das maiores propostas e descobertas para os principais mecanismos patogénicos e novas abordagens terapêuticas desta patologia.Métodos: Foram consultadas as bases de dados PubMed e Scopus, limitando-se aos artigos publicados entre janeiro/2014 e março/2024. De entre os resultados obtidos, foi feita uma seleção com base na pertinência do seu tema e na relevância dos mesmos junto da comunidade científica. Foi também consultado o relatório de 2022 do Gabinete de Registo Nacional de Biópsias Renais, da Sociedade Portuguesa de Nefrologia.Discussão: O modelo “multi-hit” permanece como consenso explicativo da patogénese, porém, fatores genéticos, a interação proposta de um eixo mucosa-rim e a ativação da via alternativa e da lectina do sistema do complemento, são agora fatores com envolvimento fulcral para a indução de lesão renal. Não existem biomarcadores específicos de doença validados e a realização de biópsia renal permanece o único método para estabelecer diagnóstico definitivo, permitindo verificar a presença de IgA como imunoglobulina dominante ou codominante em depósitos mesangiais, em microscopia de imunofluorescência. Esses depósitos produzem respostas clínicas variáveis, porém, a apresentação clínica mais comum é de hematúria microscópica associada a graus variados de proteinúria e de lesão renal crónica progressiva. A classificação de Oxford MEST-C e ferramentas como a IIgAN-PT são fundamentais para prever prognóstico, contudo, não permitem ainda guiar tratamento. O tratamento de suporte mantém-se central na gestão clínica de todos os doentes com IgAN primária, tendo como objetivo atrasar a progressão da doença renal crónica. Para doentes a exibir alto risco de progressão, é recomendado considerar um ciclo de imunossupressão sistémica (corticosteroide sistémico e/ou micofenolato de mofetil), com efeitos secundários não desprezíveis, tendo sido aprovada uma alternativa terapêutica consistindo numa formulação entérica de budesonida. Em doentes não sujeitos ou suscetíveis a imunossupressão, que mantêm proteinúria persistente após tratamento de suporte otimizado com IECA/ARAII, novas abordagens foram aprovadas, como associação de iSGLT2 ou substituição por Sparsentan. Novos agentes, como modeladores do complemento e agentes dirigidos a linfócitos B, encontram-se em ensaios clínicos, com resultados promissores.Conclusões: As novas abordagens em estudo, baseadas no maior entendimento da patogénese da IgAN, levam a acreditar que o futuro trará alternativas válidas, com estratégias efetivas a abordar diferentes intervenientes patogénicos desta doença imunológica crónica, de forma individualizada. Essa individualização constitui o grande desafio na abordagem dos doentes com IgAN, aguardando-se a descoberta de biomarcadores específicos de doença que permitam guiar decisões terapêuticas de forma fidedigna, segura e reprodutível.
