Abstract

A Displasia Fibrosa (DF) é uma displasia óssea causada por mutações no gene GNAS em mosaico, levando a deformidades ósseas, fraturas e dor. Pode ocorrer associada a manifestações extra-esqueléticas, forma designada por síndrome de McCune-Albright (SMA).Foi realizado um estudo descritivo retrospetivo dos doentes com DF Poliostótica/SMA, seguidos na ULS Coimbra (2015-2023), com o objetivo de caracterizar a população seguida e servir como base para otimização de cuidados.Dos 101 casos identificados com DF, este estudo focou-se em 12 casos (7M;5F), 8 com DFP e 4 com SMA, com idade mediana ao diagnóstico de 7.5 anos (5 dias - 31 anos) e idade mediana à última avaliação de 16 anos (9 - 38 anos).Como sintomatologia inaugural mais comum, 5 (41,6%) apresentaram fraturas, 3 (25%) anomalias craniofaciais (CF) e 2 (16.6%) dor/claudicação. À data da última avaliação, 11 indivíduos apresentavam lesões nos membros inferiores, 7 (58.3%) na bacia e 6 (50%) lesões CF. Destes indivíduos, 7 (58.3%) já tinham apresentado fraturas e sido submetidos a tratamento cirúrgico; 7 (58.3%) realizaram biópsia; 10 (83.3%) cintigrafia; e 5 tratamento com bifosfonatos. A nível analítico, as alterações mais frequentes foram o défice de vitamina D em 8 (66.6%) casos e o aumento da fosfatase alcalina e hipofosfatémia em 5 (41.6%).Das manifestações extra-esqueléticas, alterações dermatológicas foram identificadas em 8 (66.6%), mas as típicas manchas café-com-leite apenas em 3 casos com SMA. Foram observados ainda: puberdade precoce (n=2); quistos testiculares/ováricos (n=3); nódulos tiroideus (n=2); adenoma da hipófise (n=1); hiperandroginismo (n=1); e hiperprolactinemia (n=1).Este estudo permitiu uma melhor compreensão das necessidades diagnósticas e de cuidados multidisciplinares. Nove (75%) casos beneficiaram de proposta de medidas de otimização do seu seguimento.Esta dissertação inclui ainda a descrição de um caso clínico e discussão do papel da PET/CT Na[18F]F no diagnóstico e monitorização da atividade da doença.

Fibrous Dysplasia (DF) is a bone dysplasia caused by mosaic mutations in the GNAS gene, leading to bone deformities, fractures and pain. It can occur in association with extraskeletal features, comprising the McCune-Albright Syndrome (SMA).A descriptive and retrospective study of patients with Polyostotic DF/SMA followed in ULS Coimbra (2015-2023) was performed, to characterize the population being followed and as basis for the optimization of care.Of the 101 cases of DF identified, this analysis focused on 12 cases (7M;5F), 8 with DFP and 4 with SMA, with a median age at diagnosis of 7.5 years (5 days - 31 years) and median age at the last evaluation of 16 years (9 - 38 years).As most common inaugural symptoms, 5 (41,6%) patients presented with fractures, 3 (25%) craniofacial anomalies (CF) and 2 (16.6%) pain/claudication. At the last evaluation, 11 presented lesions in the lower limbs, 7 (58.3%) in the pelvis and 6 (50%) craniofacial lesions. Of these, 7 (58.3%) had already presented fractures and underwent surgery; 7 (58.3%) biopsies; 10 (83.3%) bone scintigraphs and 5 treatment with bisphosphonates. The most frequent analytical abnormalities were low vitamin D in 8 (66.6%) cases and high alkaline phosphatase and hypophosphatemia in 5 (41.6%).Of extraskeletal features, dermatological alterations were identified in 8 (66.6%), but only 3 cases with SMA presented typical café-au-lait macules. Were also observed: precocious puberty (n=2); testicular/ovarian cysts (n=3); thyroid nodules (n=2); pituitary adenoma (n=1); hyperandrogenism (n=1); and hyperprolactinemia (n=1).This study allowed a better comprehension of diagnostic needs and multidisciplinary care. Nine (75%) cases benefited with the proposal of measures to optimize follow-up.This dissertation includes the description of a case report and discussion on the use of PET/CT Na[18F]F in the diagnosis and monitoring of disease activity.
