Abstract

O tratamento da dor no doente queimado assenta numa abordagem multimodal, sendo os opióides os fármacos mais utilizados. Após a alta hospitalar, pode haver desenvolvimento de dor crónica e/ou patologia psiquiátrica. Todos estes aspetos culminam em importantes consequências na saúde e num considerável impacto socioeconómico. Os objetivos deste estudo são comparar os esquemas analgésicos relativamente à sua eficácia, assim como identificar fatores preditores de mortalidade e desenvolvimento de dor crónica. O estudo foi dividido em duas partes: na primeira analisaram-se os dados clínicos dos doentes internados na Unidade de Queimados da Unidade Local de Saúde de Coimbra num período de 3 anos e meio; e na segunda realizaram-se questionários a um subgrupo desses doentes após a alta. Não foram identificados fatores preditores associados ao controlo da dor (onde se incluíram os diferentes esquemas analgésicos), através de uma regressão logística ordinal, pelo que deverá ser adotado um tratamento analgésico individualizado e personalizado. Os fatores preditores de mortalidade encontrados foram a idade (OR 1,044), a superfície corporal queimada (SCQ) (OR 1,085), ambos inseridos no score de Baux, e o númerode dias em ventilação mecânica invasiva (OR 1,048), através de uma regressão logística binária. O maior número de sessões de balneoterapia (OR 0,668) está associado a menor mortalidade, pela provável diminuição do risco infecioso. O único fator preditor de desenvolvimento de dor crónica foi a idade mais jovem (OR 0,95), contrariamente ao expectável.

PAIN MANAGEMENT IN A BURN CARE UNIT, RENATO BORGES: Pain management in burn patients relies on a multimodal approach, with opioids being the most commonly used drugs. After hospital discharge, the patients can develop chronic pain and/or psychiatric disorders. All these aspects culminate in a significant illness burden, with considerable socioeconomic impact. The goals of the study are to compare analgesic regimens regarding their effectiveness, as well as to identify predictors of mortality and chronic pain development. The study was divided into two parts: in the first part, clinical data of patients admitted to the Burn Unit of Unidade Local de Saúde de Coimbra over a period of 3 and a half years was analyzed; and in the second part, questionnaires were conducted to a subgroup of these patients after discharge. No predictors associated with pain control (including different analgesic regimens) were identified through ordinal logistic regression, indicating that individualized and personalized analgesic treatment should be adopted. Predictors of mortality were identified as age (OR 1.044), total burn surface area (OR 1.085), both included in the Baux score, and the number of days on invasive mechanical ventilation (OR 1.048), through binary logistic regression. The number of hydrotherapy sessions (OR 0.668) is associated with lower mortality, probably due to decreased infectious risk. The only predictor of chronic pain development was younger age (OR 0.95), contrary to expectations.
