Abstract

Introdução: Ainda que a colangiopancreatografia retrógrada endoscópica (CPRE) seja uma técnica relativamente segura, a incidência de complicações pós-CPRE é superior relativamente às dos restantes procedimentos endoscópicos. Note-se que os fatores de risco para colangite pós-CPRE não estão bem definidos e que esta complicação é particularmente importante pela crescente preocupação com as infeções relacionadas com o duodenoscópio. Portanto, o objetivo do nosso estudo é analisar a epidemiologia e caracterizar os fatores de risco para o desenvolvimento de colangite pós-CPRE, assim como identificar possíveis estratégias de mitigação.Materiais e métodos: Estudo de coorte retrospetivo e observacional que incluiu 327 CPRE não consecutivas, realizadas entre Janeiro de 2021 e Dezembro de 2022 no Serviço de Gastrenterologia do Centro Hospitalar e Universitário de Coimbra. Analisámos vários fatores relacionados com o doente e com o procedimento como possíveis fatores de risco para colangite pós-CPRE. Efetuámos a análise bivariada e multivariada para identificar fatores de risco independentes para esta complicação.Resultados: A incidência global de complicações foi de 20,2%. Ocorreram 15 casos (4,6%) de colangite pós-CPRE. Na maioria dos casos (60,0%), o diagnóstico de colangite pós-CPRE foi realizado no primeiro dia pós-CPRE. A idade média dos doentes com colangite pós-CPRE foi de 68,1 +/- 12,0 anos e dois terços (66,7%) eram do sexo masculino. A análise bivariada demonstrou que a colangite esclerosante primária (CEP) foi fator de risco e a profilaxia antibiótica fator protetor para colangite pós-CPRE (p<0,050). Na análise multivariada verificou-se que a CEP e a colangioscopia foram fatores de risco independentes para colangite pós-CPRE (p<0,050) e não foram identificados fatores com potencial protetor.Conclusão: Concluímos que a CEP e a colangioscopia foram fatores de risco independentes para colangite pós-CPRE. A profilaxia antibiótica deve ser considerada em determinadas circunstâncias.

Introduction: Even though endoscopic retrograde cholangiopancreatography (ERCP) is a relatively safe technic, the rate of post-ERCP complications is higher when compared to other endoscopic procedures. Note that the risk factors for post-ERPC cholangitis are not well defined and that this complication is particularly important due to the rising concern about infections related to the duodenoscope. Therefore, the goal of our study is to analyse the epidemiology and characterise the risk factors for the development of post-ERPC cholangitis, as well as to identify possible mitigation strategies.Materials and methods: Retrospective and observational cohort study which included 327 non-consecutive ERPC, performed between January 2021 and December 2022 in the Gastroenterology Department of Centro Hospitalar e Universitário de Coimbra. We analysed multiple factors related to the patient and the procedure as possible risk factors for post-ERPC cholangitis. We performed a bivariate and multivariate analysis to identify independent risk factors for this complication.Results: The incidence rate for complications was 20.2%. There were 15 cases (4.6%) of post-ERPC cholangitis. The mean age of the patients with post-ERCP cholangitis was 68,1 +/- 12,0 years and two-thirds (66,7%) were male. In most cases (60,0%), post-ERPC cholangitis was diagnosed on the first-day post-ERPC. The bivariate analysis demonstrated that primary sclerosing cholangitis (PSC) was a risk factor and that antibiotic prophylaxis was a protective factor for post-ERPC cholangitis (p<0.050). In the multivariate analysis we verified that the PSC and the cholangioscopy were independent risk factors for post-ERPC cholangitis (p<0,050) and no potential protective factors were identified.Conclusion: We concluded that PSC and cholangioscopy were independent risk factors for post-ERPC cholangitis. Antibiotic prophylaxis should be considered in some cases.
