Abstract

Introduction: Cutaneous squamous cell carcinoma is the second most prevalent cutaneousmalignant neoplasm and one that has shown an increasing overall incidence. It usually has agood prognosis, but some cases carry a significant risk of mortality. Mohs micrographicsurgery, is a technique used in the treatment of various skin tumors, including cutaneoussquamous cell carcinoma, that allows high cure rates and preservation of healthy tissues. Thisreview aims to briefly address the history and technical principles of Mohs surgery, review itsindications and contraindications, analyze the advantages and disadvantages of its applicationin the treatment of cutaneous squamous cell carcinoma, comparing it with conventionalsurgical excision regarding efficacy, safety, preservation of healthy tissues, costs, costeffectiveness and impact on patient quality of life.Methods: A literature search was conducted in MEDLINE and PubMed databases, usingkeywords related to cutaneous squamous cell carcinoma and Mohs surgery to create a searchequation. After applying inclusion and exclusion criteria, the articles were selected based onreading the respective titles and abstracts. The most current international guidelines on thesubject were also considered.Body: The choice of the most appropriate surgical treatment depends on the risk stratificationof each case, based on tumor and patient characteristics. The Mohs technique has evolvedsince the 1930s and involves several steps: tumor delimitation and excision; resection of a thinlayer along the margins; mapping and processing of the frozen sections and microscopicexamination of the slides. These are repeated until negative margins are obtained. Mohssurgery is indicated in cases of cutaneous squamous cell carcinoma with high-risk features, incases of incomplete excision and in the treatment of specific patient groups such as theimmunosuppressed. Compared to conventional surgical excision, Mohs surgery has highercure rates and appears to be more cost-effective. While both techniques are safe, Mohssurgery has less aesthetic and functional impact and provides greater patient satisfaction.Discussion and conclusion: Mohs surgery ensures a lower risk of recurrence, maximizesthe preservation of function and aesthetics of healthy tissues, and has a positive impact on thequality of life of patients with cutaneous squamous cell carcinoma. Further research is needed,in particular, randomized controlled trials to more solidly define the indications for surgicaltreatment of cutaneous squamous cell carcinoma.

Introdução: O carcinoma espinocelular cutâneo (CECc) é a segunda neoplasia cutâneamaligna mais prevalente e que tem apresentado uma incidência global crescente. Geralmentetem um bom prognóstico, mas alguns casos comportam um risco significativo de mortalidade.A cirurgia micrográfica de Mohs (CMM), é uma técnica usada no tratamento de vários tumorescutâneos, entre os quais o CECc, que permite altas taxas de cura e preservação de tecidossaudáveis. Esta revisão tem como objetivos abordar sucintamente a história e os princípiostécnicos da CMM, rever as suas indicações e contraindicações, analisar as vantagens edesvantagens da sua aplicação no tratamento do CECc, comparando-a com a excisãocirúrgica convencional (ECC) quanto à eficácia, segurança, preservação dos tecidossaudáveis, custos, relação custo-efetividade e impacto na qualidade de vida dos doentes.Métodos: Foi realizada uma pesquisa bibliográfica nas bases de dados MEDLINE e PubMed,utilizando palavras-chave relacionadas com CECc e CMM para criar uma equação depesquisa. Após a aplicação de critérios de inclusão e exclusão, a seleção dos artigos foiefetuada com base na leitura dos respetivos títulos e resumos. Também foram consideradasas guidelines internacionais mais atuais sobre o tema.Desenvolvimento: A escolha do tratamento cirúrgico mais adequado depende daestratificação de risco de cada caso, com base nas características do tumor e do doente. Atécnica da CMM evoluiu desde a década de 30 e envolve várias etapas: delimitação e excisãotumoral; resseção de uma camada fina ao longo das margens; mapeamento e processamentodos cortes de congelação e exame microscópico das lâminas. Estas são repetidas até que seobtenham margens negativas. A CMM está indicada nos casos de CECc com característicasde alto risco, nos casos de excisão incompleta e no tratamento de grupos de doentesespecíficos, como os imunodeprimidos. Em comparação com a ECC, a CMM apresenta taxasde cura superiores e aparenta ser mais custo-efetiva. Embora ambas as técnicas sejamseguras, a CMM tem um menor impacto estético e funcional e proporciona uma maiorsatisfação dos doentes.Discussão e Conclusão: A CMM assegura um menor risco de recorrência, maximiza apreservação da função e estética dos tecidos saudáveis e tem impacto positivo na qualidadede vida dos doentes com CECc. É necessária mais investigação, em especial, ensaios clínicoscontrolados e randomizados para que se possa definir com maior solidez as indicações dotratamento cirúrgico do CECc.
