Introdução: O carcinoma esofágico continua a ser uma das neoplasias mais agressivas e com pior prognóstico relativamente à sobrevida global (SG) e livre de doença (SLD). Uma panóplia variada de estudos tem sido realizados no âmbito de perceber que fatores permitem inferir acerca do prognóstico destes doentes, sendo que a margem de resseção radial (MR) continua a ser um dos fatores com resultado controverso. Visto que a esofagectomia continua a ser a modalidade terapêutica gold-standard, pretende-se com este estudo perceber o impacto da MR na recidiva, SLD e SG.Materiais e métodos: Estudo de coorte retrospetivo que incluiu 75 doentes submetidos a esofagectomia por carcinoma num período entre janeiro de 2008 e dezembro de 2020 no CHUC. As peças de esofagectomia foram devidamente analisadas com determinação da MR. O follow-up dos doentes e a documentação de recidiva foi feita com base em exames de imagem. Resultados: Doentes com MR>1mm apresentam melhor SG que os doentes com MR1mm, confirmando o impacto prognóstico desta variável na vida dos doentes. É preponderante confirmar os resultados em estudos multicêntricos prospetivos, com uma amostra mais alargada, para um melhor tratamento e follow-up adaptado à MR. Introduction: Esophageal carcinoma remains as one of the most aggressive and worst prognosis cancers with low global (GS) and disease-free survival (DFS). A huge range of studies have been made to understand which factors may affect patient’s prognosis. Radial resection margins (RM) still a controversial one. Since esophagectomy remains the Gold-Standard treatment for patients with locally advanced carcinoma, this project intends to understand RM’s impact in disease’s relapse, DFS and GS. Materials and Methods: Retrospective cohort study that included 75 patients after esophagectomy per carcinoma form January 2008 till December 2020 at CHUC. Tumor sample was properly analyzed by specialists, with special focus on RM and staging. Follow-up and relapse documentation was made based on imageology exams. Neoadjuvant and adjuvant therapies were equally weighted. Results: Patients with RM >1mm showed better GS than those with RM1mm, which confirms the main propose of this trial, the prognostic value of RM in patients lives. Therefore, prospective studies, with a bigger sample are essential to confirm these conclusions and vital to improve treatment and follow-up of patients, after esophagectomy, adapted to RM.
