Abstract

O termo Neonatologia surge em 1960 por Alexandre Schaffer, referindo-se à subespecialidade da Pediatria responsável pelos cuidados e tratamentos ao recém-nascido definido como todo o nado vivo após clampagem do cordão umbilical até aos 28 dias de vida. Desde essa altura verificaram-se enormes progressos em termos científicos, tecnológicos e terapêuticos que revolucionaram os cuidados de saúde, com impacto particular no recém-nascido pré-termo. Para além disto, a organização da Rede de Cuidados Perinatais bem como a vontade em mudar o paradigma da Saúde Materna e Infantil em Portugal, permitiu a melhoria dos indicadores relacionados com a saúde materna, fetal, neonatal e infantil. O sucesso dos cuidados intensivos neonatais é inegável: nenhum outro programa de cuidados médicos organizado, tendo como alvo um grupo específico da população, teve um impacto tão grande nas taxas de mortalidade num período de tempo tão reduzido, com menos despesa do que a grande maioria dos países que constituíam a União Europeia. Portugal é reconhecido a nível mundial como um exemplo paradigmático desse bom desempenho. Revisitar a história da Neonatologia em Portugal permitirá compreender o estado da arte atual e servir de base para refletir sobre questões que se vão tornando mais prementes nesta área da Medicina, tais como o maior número de recém-nascidos pré-termo e de recém-nascidos muito baixo peso, o aumento da taxa de mortalidade materna, a baixa taxa de natalidade, o aumento da idade materna ao nascimento do primeiro filho, bem como novos desafios éticos e novas promessas para o futuro da Neonatologia.

The term Neonatology appears in 1960 by Alexandre Schaffer, referring to the subspecialty of Pediatrics responsible for the care and treatment of newborns defined as all live births after umbilical cord clamping up to 28 days of life. Since then, enormous advances have been made in scientific, technological and therapeutical areas that have improved medical care, with a particular impact on preterm newborns. Furthermore, the organization of the Perinatal Care Network as well as the will to change the paradigm of Maternal and Child Health in Portugal, allowed the improvement of indicators related to maternal, fetal, neonatal and child health. The success of neonatal intensive care is undeniable: no other healthcare program, targeting a specific population group, has had such a large impact on mortality rates in such a short period of time, with less expense than the vast majority of the countries that were part of the European Union. Portugal is recognized worldwide as a prime example of this good performance. Revisiting the history of Neonatology in Portugal will allow us to understand the current state of the art and allow to reflect on issues that are becoming more pressing in this area of Medicine, such as the increased number of preterm and very low birth weight newborns, the increased of the maternal mortality rate, the low birth rate, the increased maternal age at the birth of the first child, as well as new ethical challenges and new promises for the future of Neonatology.
