Diagnosis of Helicobacter pylori Infection

Abstract

L'infection à Helicobacter pylori joue un rôle important dans la pathogenèse de la gastrite chronique, de l'ulcère gastroduodénal et de la malignité gastrique. Un diagnostic d'infection est donc un élément important d'une stratégie de traitement de nombreuses maladies du tractus gastro-intestinal. De nombreux tests de diagnostic sont disponibles, mais tous ont certaines limites dans différentes situations cliniques et en laboratoire. Un étalon-or unique ne peut pas être disponible, mais doit être utilisé pour le diagnostic de l'infection à Helicobacter pylori dans la pratique clinique quotidienne dans tous les domaines, de sorte que plusieurs techniques ont été développées pour donner des résultats fiables, en particulier en se concentrant sur les caractéristiques endoscopiques en temps réel. Le système d'imagerie à bande étroite (NBI) et l'endoscopie à haute résolution sont des techniques d'imagerie pour une visualisation améliorée des muqueuses infectées et des lésions gastriques prémalignes. L'objectif de cet article est de passer en revue les options de diagnostic actuelles et les développements futurs possibles pour la détection de l'infection à Helicobacter pylori.

La infección por Helicobacter pylori juega un papel importante en la patogénesis de la gastritis crónica, la enfermedad de la úlcera péptica y la malignidad gástrica. Por lo tanto, un diagnóstico de infección es una parte importante de una estrategia de tratamiento de muchas enfermedades del tracto gastrointestinal. Hay muchas pruebas de diagnóstico disponibles, pero todas tienen algunas limitaciones en diferentes situaciones clínicas y entornos de laboratorio. No se puede disponer de un único estándar de oro, pero se puede utilizar para el diagnóstico de la infección por Helicobacter pylori en la práctica clínica diaria en todas las áreas, por lo que se han desarrollado varias técnicas para obtener resultados fiables, centrándose especialmente en las características endoscópicas en tiempo real. El sistema de imágenes de banda estrecha (NBI) y la endoscopia de alta resolución son técnicas de imagen para mejorar la visualización de la mucosa infectada y las lesiones gástricas premalignas. El objetivo de este artículo es revisar las opciones de diagnóstico actuales y los posibles desarrollos futuros en la detección de la infección por Helicobacter pylori.

Helicobacter pylori infection plays an important role in the pathogenesis of chronic gastritis, peptic ulcer disease and gastric malignancy. A diagnosis of infection is thus an important part of a treatment strategy of many gastrointestinal tract diseases. Many diagnostic tests are available but all have some limitations in different clinical situations and laboratory settings. A single gold standard cannot available, but be used for diagnosis of Helicobacter pylori infection in daily clinical practice in all areas, so several techniques have been developed to give reliable results, especially focusing on real time endoscopic features. The narrow band imaging system (NBI) and high resolution endoscopy are imaging techniques for enhanced visualization of infected mucosa and premalignant gastric lesions. The aim of this article is to review the current diagnostic options and possible future developments detection of Helicobacter pylori infection.

تلعب عدوى الملوية البوابية دورًا مهمًا في التسبب في التهاب المعدة المزمن ومرض القرحة الهضمية والأورام الخبيثة في المعدة. وبالتالي فإن تشخيص العدوى هو جزء مهم من استراتيجية علاج العديد من أمراض الجهاز الهضمي. تتوفر العديد من الاختبارات التشخيصية ولكن جميعها لها بعض القيود في المواقف السريرية المختلفة والإعدادات المختبرية. لا يمكن توفير معيار ذهبي واحد، ولكن يمكن استخدامه لتشخيص عدوى الملوية البوابية في الممارسة السريرية اليومية في جميع المجالات، لذلك تم تطوير العديد من التقنيات لإعطاء نتائج موثوقة، وخاصة التركيز على ميزات التنظير في الوقت الحقيقي. يعد نظام التصوير ضيق النطاق (NBI) والتنظير عالي الدقة من تقنيات التصوير لتعزيز رؤية الغشاء المخاطي المصاب وآفات المعدة قبل الخبيثة. الهدف من هذه المقالة هو مراجعة خيارات التشخيص الحالية والتطورات المستقبلية المحتملة للكشف عن عدوى الملوية البوابية.
