Eficacia de la ortodoncia lingual vs convencional. Revision sistemática [Effectiveness of lingual orthodontics versus conventional orthodontics. Systematic review]

Abstract

Objetivo: comparar ortodoncia convencional versus ortodoncia lingual analizando efectividad, tiempo de tratamiento, comodidad del paciente, estética y relación costo-beneficio. Método: revisión bibliográfica siguiendo metodología PRISMA. Se analizaron 16 artículos de bases de datos SCIELO, PubMed, Elsevier, ResearchGate, Redalyc, Dialnet y Dental Update, seleccionados de 70 artículos inicialmente identificados. Resultados: Ambas técnicas demostraron eficacia similar en corrección de maloclusiones. La ortodoncia lingual ofrece ventajas estéticas significativas al ubicarse en cara interna de dientes, siendo preferida por pacientes adultos. Sin embargo, presenta mayor incomodidad inicial, dificultades en pronunciación y requiere mayor experiencia del ortodoncista. El tiempo de tratamiento puede ser más prolongado en ortodoncia lingual debido a complejidad técnica, aunque sistemas personalizados CAD/CAM han mejorado biomecánica. La ortodoncia convencional mantiene mayor accesibilidad económica, menor complejidad clínica, mejor control del torque y menor fricción. Los avances en brackets autoligables y sistemas de baja fricción han optimizado ambas técnicas.
