Quadricuspid aortic valve associated with aortic insufficiency.
33歳,男性,大動脈弁閉鎖不全.手術時,大動脈4尖弁であることが判明,右冠尖と左冠尖の間に過剰尖があり,この三つはほぼ同大,無冠尖が小さかった.Bjork-Shiley弁で置換,術後経過は良好である.大動脈4尖弁はまれで,報告は39例,大多数が成人になってから大動脈弁閉鎖不全となり,手術報告は9例である.最近,大動脈造影,心エコーにより診断可能となった.文献例の調査により,本症を過剰尖の位置により2群にわけることを提案した,I型は過剰尖が右冠尖と左冠尖の間,すなわち前方にあり,報告例はこれに属する.II型は過剰尖が右冠尖と無冠尖の間,すなわち後方にあり,この場合は全例,過剰尖が小さい.
