Factores asociados a hemorragia materna por atonía uterina en postparto inmediato

Abstract

Objetivo: Identificar y analizar los factores asociados a la hemorragia materna por atonía uterina en el postparto inmediato para mejorar la prevención y manejo de esta complicación obstétrica. Método: Estudio cualitativo mediante investigación documental no experimental con alcance descriptivo. Se realizó revisión sistemática de literatura publicada entre 2018-2023 en bases de datos Google Académico, Dialnet, Scielo, Reciamuc y repositorios universitarios, utilizando términos "hemorragia postparto", "atonía uterina" y "factores de riesgo". Se analizaron 62 bibliografías, seleccionando 22 artículos que cumplieron criterios de inclusión. Resultados: Los principales factores asociados incluyen distensión uterina por embarazo múltiple, polihidramnios y macrosomía fetal; agotamiento muscular por trabajo de parto prolongado; inducción con oxitocina (84.7%); tipo de parto (cesárea 67-70%); edad materna mayor de 35 años (79%); multiparidad (63.6%); y corioamnionitis. Conclusiones: La identificación temprana de factores de riesgo durante control prenatal y aplicación de medidas preventivas mediante protocolos estandarizados reduce significativamente la morbimortalidad materna.
