Perturbação de Dor

Abstract

A perturbação de dor é uma perturbação psiquiátrica diagnosticada quando a dor se torna o foco predominante da apresentação clínica e causa sofrimento ou deterioração significativa. Para além do elevado impacto económico, existe uma relação recíproca com o estado afectivo. A dor é uma sensação subjectiva e a sua gravidade e qualidade como experiência num indivíduo é dependente de uma complexa mistura de factores. No tratamento da dor aguda, o objectivo primário é o alívio da dor, enquanto que a dor crónica geralmente requer uma associação de psicoterapia com medicação. Neste contexto, também é importante reconhecer e tratar a depressão. Os tratamentos psicológicos visam dotar o doente de mecanismos para controlar e "viver com a dor", em vez de aspirar a eliminá-la completamente. Actualmente, um grupo crescente de investigadores propõe a eliminação do capítulo das Perturbações Somatoformes e a modificação da categoria "factores psicológicos afectando uma condição médica" para "factores psicológicos afectando uma condição médica identificada ou receada" com entidades clínicas como subcapítulos, largamente baseadas nos Critérios de Diagnóstico para a Investigação Psicossomática.
