A Rare Occurrence of Focal Atrial Tachycardia in a Patient with Mixed Connective Tissue Disease and Chronic Rheumatic Heart Disease: A Possible Linkage?

Abstract

La tachycardie auriculaire focale (TA) est une forme relativement rare d'arythmie. 10% de la tachycardie supraventriculaire est due à la TA. Elle peut être diagnostiquée chez un patient sans maladie cardiaque structurelle, mais chez les personnes âgées, une maladie cardiovasculaire est souvent présente. Nous rapportons une rare occurrence de tachycardie auriculaire focale, prouvée sur une cartographie électrophysiologique tridimensionnelle (3D) du cœur chez une femme de 44 ans présentant une maladie sous-jacente du tissu conjonctif mixte et une maladie cardiaque rhumatismale chronique qui a été traitée avec succès par ablation par radiofréquence. La question principale restait sans réponse de savoir si l'arythmie pouvait être liée à sa maladie sous-jacente ou à une découverte purement fortuite.

La taquicardia auricular focal (TA) es una forma relativamente poco común de arritmia.10% de la taquicardia supraventricular se debe a la TA. Se puede diagnosticar en un paciente sin ninguna enfermedad cardíaca estructural, sin embargo, en los ancianos, la enfermedad cardiovascular a menudo está presente. Reportamos una rara aparición de taquicardia auricular focal, probada en un mapeo electrofisiológico tridimensional (3D) del corazón en una mujer de 44 años con enfermedad subyacente del tejido conectivo mixto y enfermedad cardíaca reumática crónica que fue tratada con éxito con ablación por radiofrecuencia. La pregunta primaria sigue sin respuesta si la arritmia podría estar relacionada con su enfermedad subyacente, o simplemente un hallazgo coincidente.

Focal atrial tachycardia (AT) is a relatively uncommon form of arrhythmia.10% of supraventricular tachycardia are due to AT.It can be diagnosed in a patient without any structural heart disease, however in the elderly, cardiovascular disease is often present.We report a rare occurence of focal atrial tachycardia, proven on three dimensional (3D) electrophysiology mapping of the heart in a 44-years-old woman with underlying mixed connective tissue disease and chronic rheumatic heart disease who was successfully treated with radiofrequency ablation.Primary question remained unanswered whether the arrythmia could be linked to her underlying disease, or purely a coincidental finding.

عدم انتظام دقات القلب الأذيني البؤري (AT) هو شكل غير شائع نسبيًا من عدم انتظام ضربات القلب. 10 ٪ من عدم انتظام دقات القلب فوق البطيني ناتج عن AT. يمكن تشخيصه في مريض بدون أي مرض قلبي هيكلي، ولكن في كبار السن، غالبًا ما يكون مرض القلب والأوعية الدموية موجودًا. نبلغ عن حدوث نادر لعدم انتظام دقات القلب الأذيني البؤري، مثبت على خرائط الفيزيولوجيا الكهربية ثلاثية الأبعاد (3D) للقلب في امرأة تبلغ من العمر 44 عامًا تعاني من مرض النسيج الضام الكامن وأمراض القلب الروماتيزمية المزمنة التي عولجت بنجاح باستئصال الترددات الراديوية. بقي السؤال الأولي دون إجابة عما إذا كان يمكن ربط عدم انتظام دقات القلب بمرضها الكامن، أو مجرد نتيجة عرضية.
