Abstract

Le evidenze derivate dal mondo reale, basate su database elettronici sanitari, registri di malattia o di prodotto e studi osservazionali, sono fra i temi più discussi nell’ambito dei congressi medici. Al fine di facilitare l’approvazione di nuove terapie e di nuovi dispositivi, le evidenze dal mondo reale si rivelano un utile strumento a supporto dei dati raccolti a scopo regolatorio, anche se i trial randomizzati controllati restano il gold standard della ricerca clinica per ridurre al minimo il rischio di bias e valutare l’efficacia di un determinato trattamento/intervento. Nell’ottica di ridurne la durata e i costi degli studi si potrebbe prevedere un approccio pragmatico mediante la conduzione di trial semi-randomizzati. Il problema risiede nella qualità dei dati piuttosto che nel contesto in cui sono raccolti.
