Luxación congénita de cadera

Abstract

La frecuencia de la luxacion congenita de cadera es del 6-20 por 1.000 nacidos vivos, por lo que se trata de un problema de pediatria corriente. La teoria de la «postura luxante» intrauterina explica la aparicion de esta patologia durante la gestacion. Los factores geneticos, en particular una hiperlaxitud articular, son elementos predisponentes. La luxacion coxofemoral provoca una displasia acetabular de topografia posterosuperior. La deteccion precoz durante los primeros meses de vida se basa en la exploracion fisica, mediante la que se evalua la abduccion de la cadera y se busca una inestabilidad. La ecografia, que se realiza a los 1-2 meses de edad, sirve de ayuda para la deteccion precoz. La radiografia, que se realiza a los 4 meses de edad, es el ultimo bastion de la deteccion precoz. Estas exploraciones radiologicas se realizan si existen factores de riesgo, pero dos tercios de las luxaciones se producen sin que haya estos factores. Se puede realizar una artrografia o una resonancia magnetica (RM) en estos casos particulares. La luxacion se puede reducir de forma ortopedica, utilizando metodos ambulatorios u otros que requieren hospitalizacion. La reduccion quirurgica esta indicada cuando fracasan los metodos ortopedicos. Las osteotomias pelvicas y/o femorales permiten, cuando es preciso, corregir las anomalias de orientacion osea. Es necesario realizar un seguimiento periodico durante todo el crecimiento.
