espite remarkable success in the management of craniopharyngiomas in recent decades, some problems still D need to be specified and solved. One problem is development of adequate principles of treatment of craniopharyngiomas spreading into the third ventricle. Surgical treatment of craniopharyngiomashas becomeoneof the spheres ofmy interest since 1971, when I started systematically to use a microsurgical technique (microscopes with special equipment). Since that time, my experience has included >1500 surgical operations on craniopharyngiomas in different locations. In recent years, 110e130 patientswith craniopharyngiomas have been operated on annually at our institute. A summary of this experience in detail is beyond the scope of this article; however, the experience allows me to formulate some principles of treatment of craniopharyngiomas in different locations and of different structures.
