Neurofibrome myxoïde du pouce

Abstract

Resume Introduction Le neurofibrome myxoide (NM) est une tumeur benigne d’origine perinerveuse peu connue. Nous rapportons un cas de NM du pouce et nous discuterons les principaux diagnostics differentiels. Observation Une femme âgee de 34 ans consultait pour une tumeur du pouce gauche evoluant depuis six mois recidivant 26 ans apres une premiere exerese. L’examen clinique trouvait une tumeur sur la face anterieure du pouce, indolore, ferme, de 4 cm par 3, de couleur peau normale, ulceree sur sa partie externe et parcourue de fines telangiectasies. La radiographie de la phalange ne montrait pas d’alteration osseuse sous-jacente. L’etude histopathologique et immuno-histochimique d’une biopsie cutanee concluait a un NM. L’exerese de la lesion etait pratiquee avec conservation de l’appareil ungueal. Aucun signe de neurofibromatose 1 n’a ete note ni chez la patiente ni chez sa famille. L’evolution etait favorable et il n’etait pas note de recidive apres 18 mois de suivi. Commentaires Les localisations habituelles du NM sont le visage, les epaules, les bras et les regions peri-ungueales. Il est habituellement une lesion solitaire, cependant, il peut etre multiple ou reapparaitre apres une premiere exerese incomplete comme chez notre patiente. Il faut le considerer dans les diagnostics differentiels d’une tumeur des extremites. Nous rapportons ce cas pour la rarete a la fois de cette tumeur et de sa localisation.
