This is a case report of a newborn infant with neonatal diabetes mellitus who showed evidence of marked hyperglycemia at 32 hrs of age. Immunoassay of serum insulin was markedly inappropriate for the elevated blood glucose concentration. The B-cells in the islets of Langerhans were reduced in number and in their content of cytoplasmic granules. Diagnostic aids and treatment are discussed.
