Trendovi u propisivanju benzodiazepina u Republici Hrvatskoj

Abstract

Benzodiazepines are medications that are frequently prescribed, especially among the elderly population, regardless of the known risks associated with their use and despite guidelines recommending their short-term application. There is an observed increasing trend of benzodiazepine consumption in Croatia. Benzodiazepines belong to the therapeutic group N05-Psycholeptics according to the ATC classification, which for the past 15 years has consistently been within the top three groups in terms of total annual drug consumption in Croatia. Due to their amnestic, hypnotic, and sedative effects, they are most commonly used for treating anxiety and related psychiatric disorders, as well as medical conditions associated with brain pathology, such as epileptic seizures. Benzodiazepines are beneficial and effective, but they come with certain risks, including: the potential for misuse, the potential for developing addiction, various side effects, and the potential for dangerous interactions with other medications. The most frequently prescribed benzodiazepines in Croatia are diazepam and alprazolam, followed by the less common oxazepam, lorazepam, and nitrazepam. In Croatia, there is an urgent public health issue concerning the rationalization of pharmacotherapy and regular monitoring of drug consumption. Croatia ranks among the top in Europe in benzodiazepine consumption and has the worst year-on-year growth trend of all OECD countries. At the local government unit level, Koprivnica-Križevci, Vukovar-Srijem, and Osijek-Baranja counties lead in terms of DDD/TSD values, while Istria, Zagreb, and Međimurje counties have had the lowest values for the past 15 years. According to age and gender analysis, studies conducted in Europe, including those in Croatia, show that the likelihood of using benzodiazepines increases with age, is higher in females, and in individuals who are already using other medications.

Benzodiazepini su lijekovi koji se često propisuju, osobito među starijom populacijom, bez obzira na činjenicu kako su poznati rizici koji su povezani s njihovom upotrebom te unatoč smjernicama koje preporučuju njihovu kratkotrajnu primjenu. Uočava se rastući trend potrošnje benzodiazepina u Hrvatskoj. Benzodiazepini spadaju u terapijsku skupinu N05-Psiholeptici prema ATK klasifikaciji, koja se u posljednjih 15 godina nalazi uvijek unutar prve tri skupine prema ukupnoj godišnjoj potrošnji lijekova u Hrvatskoj. Zbog svojeg amnestičkog, hipnotičkog i sedativnog učinka najčešće se koriste za liječenje anksioznosti i srodnih psihijatrijskih poremećaja, kao i medicinskih stanja povezanih s patologijom mozga poput epileptičkih napadaja. Benzodiazepini su korisni i učinkoviti, ali s njima dolaze i određeni rizici, uključujući: potencijal za zloupotrebu, potencijal za razvoj ovisnosti, razne nuspojave i potencijal za opasnu interakciju s drugim lijekovima. Najčešće propisivani benzodiazepini u RH su diazepam i alprazolam, njih slijede još ne tako česti oksazepam, lorazepam i nitrazepam. U RH postoji gorući javnozdravstveni problem za racionalizacijom farmakoterapije i redovitim praćenjem potrošnje lijekova. Hrvatska se nalazi u samom vrhu u Europi po potrošnji benzodiazepina i posjeduje najgori trend rasta potrošnje iz godine u godinu od svih OECD zemalja. Na razini jedinica lokalne samouprave Koprivničko-križevačka, Vukovarsko-srijemska i Osječko-baranjska županija prednjače po vrijednostima DDD/TSD, dok Istarska, Zagrebačka i Međimurska županija imaju najniže vrijednosti posljednjih 15 godina. Sukladno dobno-spolnoj analizi istraživanja provedena u Europi uključujući i ona provedena u Hrvatskoj pokazuju da vjerojatnost korištenja benzodiazepina raste s dobi, veća je u osoba ženskog spola i u osoba koje već koriste i neke druge lijekove.
