Сочетание артериальной гипертензии и сахарного диабета 2-го типа с ожирением

Abstract

Целью исследования было определение динамики частоты встречаемости артериальной гипертензии (АГ), сахарного диабета 2-го типа (СД) и ожирения как основного фактора риска этих заболеваний, их сочетания между собой и с некоторыми болезнями системы кровообращения у пациентов терапевтической клиники, госпитализированных в период с 2003 по 2011 г. Проведен анализ 25 451 истории болезней; к пациентам с АГ, СД 2-го типа, ожирением, ишемической болезнью сердца, нарушениями сердечного ритма и цереброваскулярными болезнями относили лиц с указанием в диагнозе нозологических форм с соответствующими кодами МКБ-10; рассчитывали частоту встречаемости за 3 трехлетних временных интервала, а также средний возраст пациентов во всех выделенных группах. Показано, что рост частоты встречаемости всех случаев АГ у пациентов в указанные временные интервалы ассоциирован с повышением частоты случаев АГ в сочетании с ожирением и АГ в сочетании с ожирением и СД 2-го типа, при этом рост частоты всех случаев СД 2-го типа также ассоциирован с повышением частоты случаев сочетания СД 2-го типа с АГ и ожирением. У женщин достоверное повышение среднего возраста в указанные временные интервалы было определено только в группах пациенток с АГ в сочетании с ожирением, а у мужчин в группах пациентов с АГ (все случаи) и с АГ в сочетании с ожирением. Наличие СД 2-го типа у пациентов с АГ мало изменяло частоту цереброваскулярных болезней относительно изолированной АГ, однако наличие ожирения повышало этот показатель.

The aim of the study was to elucidate dynamics of the incidence of arterial hypertension (AH), 2 type diabetes mellitus (DM), and obesity as the main risk factor of these conditions, combinations between them and certain circulation disorders in the patients admitted to our clinic from 2003 to 2011. We analysed 25,451 case histories. Patients were regarded as having AH, DM, obesity, coronary heart disease, cardiac rhythm disturbances and cerebovascular disorders if they had verified diagnosis of nosological forms corresponding to the respective ICD-10 codes. Their occurrence was calculated for 3 three-year periods with reference to the mean age of the patients in different groups. It was shown that increased incidence of AH in the above time periods was associated with the enhanced occurrence of combination of AH and obesity or AH, obesity and DM. Similarly, the increased incidence of DM was associated with the enhanced occurrence of combination DM, AH, and obesity. A significant increase of the mean age was documented only in women with AH and obesity and in men with AH or AH plus obesity. The presence of DM in AH patients had no appreciable effect on the frequency of cerebrovascular pathology that increased in the presence of AH with obesity.
