Abstract

The article deals with specific characteristics of group psychosocial therapy (GPT) in adolescents with schizophrenia spectrum disorders, depending on the course of disorder and the level of their social functioning. The authors distinguish three groups of adolescents: (1) close to acute course of disorder, with sudden onset and background premorbid high level of social functioning; (2) with gradual development of disorder starting with early adolescence, and gradual decrease of social functioning; (3) the disorder develops in childhood and the youngsters demonstrate obvious signs of a development disorder as well as inadequate social functioning. The authors have used the SDQ, the Goldstein social skills and the Lazarus coping test in order to measure the GPT characteristics in mentioned patient groups including analysis of changes in prosocial behavior, coping strategies and social skills development. The authors report correlations between some of the parameters studied and the level of social functioning according to the CGAS and draw conclusions concerning specific aspects of work with every group.

В статье изучены особенности проведения групповой психосоциальной терапии (ПСТ) с подростками с заболеваниями шизофренического спектра в зависимости от типа течения заболевания и уровня социального функционирования. Были выделены 3 группы подростков: с приближающимся к острому течением заболевания с внезапным началом на фоне преморбидно высокого уровня социального функционирования, 2) с постепенным развитием заболевания, начиная с раннего подросткового возраста, и постепенным снижением уровня социального функционирования, 3) с началом заболевания в детстве и выраженным нарушением развития и социального функционирования. Особенности проведения групповой ПСТ с указанными группами пациентов изучались на основе анализа динамики просоциального поведения, копинг-стратегий и формирования социальных навыков с помощью опросников SDQ, социальных навыков Гольдштейна и копинг-теста Лазаруса. Выявлены корреляционные взаимосвязи между некоторыми из изучаемых показателей и уровнем социального функционирования по CGAS. Сделаны выводы об особенностях работы с каждой из групп пациентов.
