The paper gives a finite step algorithm, with a MATLAB implementation, for the construction indicated in the title of the paper, which is a generalization of an algorithm for the special case of equal main diagonal entries outlined by \textit{G. H. Golub} and \textit{C. F. Van Loan} [Matrix computations (2nd ed., 1989; Zbl 0733.65016), p. 456].
