MANEJO CLÍNICO DA ENDOMETRIOSE: REVISÃO DA LITERATURA

Abstract

Introdução: A endometriose é uma patologia ginecológica crônica, benigna, estrogêniodependente e de natureza multifatorial que acomete preferencialmente mulheres em idade reprodutiva. Pode apresentar extensa diversidade de manifestações clínicas, contudo, podem ser encontrados casos de pacientes oligo ou assintomáticas e quadros de dor pélvica intensa. A respeito do tratamento clínico, tem objetivo o alívio dos sintomas da dor e a melhora da qualidade de vida da paciente.Ressalta-se que o foco do tratamento medicamentoso é a manipulação hormonal, ocasionando um ambiente inadequado para o crescimento e manutenção dos implantes da endometriose. Objetivo: Analisar o registro na literatura dos benefícios do manejo clínico em pacientes com diagnóstico de endometriose e que não necessitaram da abordagem cirúrgica posteriormente para resolução do quadro. Métodos: Trata-se de uma revisão integrativa da literatura, cujaa busca se deu de junho a setembro de 2023, nas bases de dados PubMed, LILACS e SCIELO, utilizando os termos Endometriose e Gerenciamento clínico. Resultados: Foram identificados 3.331 artigos e 10 selecionados para compor a presente revisão. Dentre os benefícios do manejo clínico da endometriose, destaca-se: tratamento com vitamina C e E; uso de implantes; regime flexível estendido (flexível MIB) placebo ou dienogest; psicoterapia com estimulação somatossensorial; Estimulação Elétrica Nervosa Transcutânea (TENS); tratamento complementar com eletroterapia; dois regimes de tratamento diferentes de didrogesterona; tratamento com dienogest; tratamento com estrogênio potencializado; estrogênio potencializado. Considerações finais: Dentre os benefícios do manejo clínico da endometriose destaca-se a melhora das condições mentais, diminuição das queixas álgicas, melhora da lubrificação e função sexual. O tratamento demanda de estratégias de manejo personalizadas durante a vida da paciente, objetivando a melhoria dos sintomas por meio da otimização do tratamento médico com o intuito de minimizar a necessidade de repetidas intervenções cirúrgicas.
