Abstract

Цель - определение риска развития обострений у пациентов с хронической обструктивной болезни легких (ХОБЛ) разной степени тяжести на основании изучения показателей цитокинового профиля и определения соотношения цитокинов Th1, Th17 клеток. Обследовано 112 больных (59 мужчин и 53 женщины, средний возраст 59,3±3,4 лет) с диагнозом ХОБЛ и 32 человека (24 мужчины и 18 женщин, средний возраст 52,4±3,6 лет), условно здоровых. В сыворотке крови определяли уровень интерлейкинов (IL) IL-17А, IL-21, интерферона гамма (IFNγ) и фактора некроза опухоли альфа (TNFα), обуславливающие тип иммунного ответа. Выполняли расчет соотношения цитокинов Th17/Th1 по формуле. По результату соотношения Th17/Th1 судили о типе иммунного ответа и риске развития обострений у пациентов ХОБЛ. При значении Th17/Th1 ≤ 3,89 у.е. прогнозируется благоприятное течение ХОБЛ с низким риском обострений, а при значении Th17/Th1 ≥ 3,90 у.е. – неблагоприятное прогрессирующее течение ХОБЛ с высоким риском обострений заболевания. Исследование цитокинового профиля и выявление характера иммунного ответа позволяет оптимизировать лечение, сократить количество обострений, снизить затраты на медикаментозную терапию, уменьшить количество случаев временной и стойкой нетрудоспособности у пациентов ХОБЛ. The aim of the study was to determine the risk of exacerbations in patients with chronic obstructive pulmonary disease (COPD) of varying severity. The cytokine profile was studied and the ratio of Th1, Th17 cytokines was estimated. 112 patients (59 men and 53 women, mean age 59.3 ± 3.4 years) were diagnosed with COPD and 32 people (24 men and 18 women, mean age 52.4 ± 3.6 years) are conditionally healthy. Serum levels of interleukin (IL) IL-17A, IL-21, interferon gamma (IFNγ) and tumor necrosis factor alpha (TNFα) were determined. The ratio of Th17 / Th1 cytokines was calculated by the formula. The type of immune response and the risk of exacerbations by the ratio Th17 / Th1 cells were determined. The ratio of cytokines Th17 / Th1 ≤ 3.89 cu Characterizes the favorable course of COPD with a low risk of exacerbations, Th17 / Th1 ≥ 3.90 USD. - an unfavorable progressive course of COPD with a high risk of exacerbations of the disease. The study of the cytokine profile and the identification of the nature of the immune response makes it possible to optimize treatment, reduce the number of exacerbations, reduce the cost of drug therapy, and reduce the incidence of temporary and permanent disability in COPD.
