Превентивная Медиастинотомия При Нисходящем Одонтогенном Медиастините

Abstract

Авторами проанализированы результаты хирургического лечения 50 пациентов с флегмонами шеи, осложненных гнойным медиастинитом. Было выделено 2 группы пациентов: первая – 32 пациента (64%) и вторая – 18 (36%). У первой группы имелись лучевые признаки медиастинита, у второй они отсутствовали. Пациентам первой группы оперативные вмешательства на средостении выполнены одновременно со вскрытием первичных очагов инфекции. Превентивная медиастинотомия у пациентов второй группы проведена в течение первых 24 часов, хотя рентгенологическая картина поражения клетчатки средостения отсутствовала. Отсутствие ранних лучевых признаков заинтересованности клетчатки средостения в гнойном процессе не исключает наличие нисходящего медиастинита. Authors have analysed results of surgical treatment of 50 patients with phlegmons of a neck complicated by a purulent mediastinit. 2 groups of patients were allocated: the first – 32 patients (64%) and the second – 18 (36%). The first group had radial signs of a mediastinitis, at the second they were absent. To patients of the first group operative measures on a mediastinum are executed at the same time with opening of primary centers of an infection. The preventive mediastinotomy at patients of the second group is carried out within the first 24 hours though the X-ray pattern of a lesion of a fat of a mediastinum was absent. The lack of precursory radial symptoms of interest of a fat of a mediastinum in purulent process doesn't exclude existence of the descending mediastinitis.
