Оптимизация Методики Выполнения Межтелового Спондилодеза С Использованием Винтовых Титановых Имплантатов

Abstract

Проводится обобщение опыта успешного выполнения 970 операций по поводу межтелового спондилодеза с использованием винтовых титановых имплантатов при оперативном лечении дегенеративных заболеваний пояснично-крестцового отдела позвоночника. Проанализированы 970 случаев оперативного лечения пациентов с МГ с применением межтелового спондилодеза на пояснично-крестцовом уровне винтовым титановым имплантатом производства «Дальмединструмент» (г. Хабаровск) с 2003 по 2012 гг. в нейрохирургическом отделении 1477 военно-морского клинического госпиталя (г. Владивосток). Хорошие результаты получены у 932 (96,1%), p<0,02; удовлетворительные – у 38 (3,9%) пациентов. Неудовлетворительных результатов не отмечено. В послеоперационном периоде больные, в первые сутки переводились на палатный режим, получали комплекс восстановительной терапии. Кроме того, удавалось сохранить стабильную высоту диска в оперированном сегменте, что предотвращало инклинацию суставных отростков и формированию вторичного дегенеративного стеноза. При выполнении декомпрессивно-стабилизирующего оперативного лечения дегенеративных заболеваний в поясничном отделе позвоночника использование винтовых титановых имплантатов обеспечивает проведение эффективного межтелового спондилодеза. Применение этого метода уменьшает травматизацию мягких тканей, кровопотерю, частоту осложнений, снижает сроки пребывания пациентов в стационаре, обеспечивая необходимую стабильность позвоночно-двигательного сегмента, и показывает лучшие клинические результаты, по сравнению с традиционными способами декомпрессии. Generalization of experience of successful implementation of 970 operations for interbody spondylosyndesis using screw titanium implants in the surgical treatment of degenerative diseases of the lumbosacral spine. Analyzed 970 cases of surgical treatment of patients with MG using interbody spondylosyndesis in the lumbar-sacral screw titanium implants produced by "Dalmedinstrument" (Khabarovsk) from 2003 to 2012 to the Neurosurgery Department in 1477 Naval Clinical Hospital (Vladivostok). Good results were obtained in 932 (96.1%), p <0.02; satisfactory – in 38 (3.9%) patients. Since that time, our observations were noted education hernia recurrence of instability (antelisteza, spondylolisthesis) at levels that were operated using this implant. In addition, it managed to maintain a stable height drive in the operated segment, which prevents inclination of articular processes and the formation of secondary degenerative stenosis. When performing decompressive-stabilizing surgical treatment of degenerative diseases of the lumbar spine using a screw titanium implants ensure the implementation of effective interbody spondylosyndesis. This method reduces soft tissue trauma, blood loss, incidence of complications, reduced patient length of stay in the hospital, providing needed stability spinal motion segment, and showing better clinical results than traditional decompression means.
