Abstract

La myasthenie est une maladie neurologique auto-immune affectant la jonction neuromusculaire qui se caracterise par la presence danticorps anti-recepteurs de lacetylcholine sur la plaque motrice. Ainsi il est estime que 70 % a 80 % des recepteurs a lacetylcholine (RAch) ne sont plus fonctionnels lorsque la maladie est declaree. La fatigabilite musculaire est le signe clinique principal elle est fluctuante, aggravee par leffort et diminue au repos elle atteint notamment la musculature faciale, axiale et respiratoire. Plus frequente avant 40 ans,avec un sexe ratio de 3 femmes pour 1 homme, cette pathologie touche la femme jeune, en âge de procreer. Lassociation myasthenie et grossesse impose denvisager deux problemes : dune part, linfluence de la grossesse sur levolution de la myasthenie est connue mais tout a fait imprevisible dautre part, linfluence de la myasthenie sur le deroulement et lissue de la grossesse apparaît peu importante (jugee sur lincidence de la prematurite, des fausses couches spontanees et des cesariennes). Certaines decisions doivent etre prises pour laccouchement, notamment sa programmation et lextraction instrumentale preventive.Le traitement medical par les anticholinesterasiquesqui permet de reduire les poussees, doit etre optimise pendant la grossesse, laccouchement et le post-partum. La place de lanalgesie loco-regionale en obstetrique a ete discutee dans de rares cas isoles. Le bloc moteur induit par les anesthesiques locaux est susceptible dalterer la mecanique obstetricale chez la parturiente ayant une force musculaire normale et pourrait theoriquement precipiter une situation dystocique chez la parturiente myasthenique. Il pourrait donc conduire a un taux accru de cesarienne.Cet effet est peu documente chez la parturiente myasthenique et lanalgesie peridurale bien conduite possede un rapport risque/benefice tres positif dans la pratique.
