PREPOZNAVANJE I OCJENJIVANJE NEGATIVNIH SIMPTOMA U SHIZOFRENIJI

Abstract

Sažetak Opisivanje negativnih simptoma u shizofreniji kroz istoriju imalo je svoje pionire, koje je neophodno pomenuti, kao i navesti njihove koncepte. Rejnolds je 1850. g. prvi upotrebio termine pozitivni i negativni simptomi. Pozitivni simptomi odnose se na hiperfunkciju, a negativni na nedostatak funkcije, odnosno stanje manjka ponašanja. Slijedi čuveni Krepelin sa četiri tipa shizofrenije, zatim Anderson, Karpenter i Crow koji su dali spisak simptoma koji su predstavljali negativni sindrom, a zatim dva etiološki nezavisna tipa shizofrenije (prvi tip u kojem negativni simptomi kognitivno oštećenje praktično ne postoji i drugi tip sa negativnim simptomima, kognitivnim oštećenjima). Anderson i Olsen su predložili bipolarni model shizofrenije, gdje su pozitivni i negativni simptomi na istom kontinuumu. Karpenter i saradnici navode da postoje primarni negativni simptomi koji daju deficitarni sindrom, otuda i podjela na deficitarnu i nedeficitarnu shizofreniju. Alogija, afekat, asocijalnost, anhedonija i avolucija su pet velikih kategorija negativnih simptoma koje procjenjujemo prilikom dijagnostike. U ispitivanju samo negativnih simptoma tokom poslednjih decenija, diskusija se odvijala oko dva glavna pitanja: (1) utvrđivanja negativnih simptoma kao različite dimenzije od drugih osnovnih karakteristika shizofrenije kao što su pozitivni simptomi i dezorganizovanost i od kliničkih fenomena (pažnja) pripadnost depresiji i/ili anksioznosti i (2) konceptualizacije negativnih simptoma kao elaborirane kombinacije simptoma (multidimenzionalni konstrukt) ili kao opšte (jednodimenzionalne) konstrukte.
